摘要
[目的]评价不同手术方法治疗老年股骨转子间骨折的疗效,总结内固定物的选择方法。[方法]回顾性分析本院于2005年1月～2009年12月收治的89例老年股骨转子间骨折的治疗过程和随访结果,其中应用动力髋螺钉治疗45例,动力髁螺钉治疗15例,股骨近端髓内钉治疗17例,人工股骨头置换术治疗12例。[结果]全部病例获得随访,时间6～24个月,平均18个月,优良率分别为:动力髋螺钉88.9%,动力髁螺钉93.3%,股骨近端髓内钉94.1%,股骨头置换组91.7%,疗效满意。[结论]治疗老年人股骨转子间骨折,4种手术方法各有所长,只有根据患者年龄、骨折类型、骨质疏松程度、手术耐受情况等合理选择手术方式,并注重围手术期管理,方可取得良好的临床效果。
[Objective] To evaluae the curative effect of different surgeries to remedy the intertrochanteric fractures in aged people and conclude the method of how to select the internal fixation.[Method]Eighty-nine cases of aged people＇s intertrochanteric fractures were treated in our hospital from January 2005 to December 2009,45 cases among which using DHS,15 cases using DCS,17 cases using PFN,12 cases using artificial femoral head replacement.[Result]All cases had been followed up for 6~24 months,averaging 18 months and receiving a satisfactory effect,with the acceptable rate of 88.9% to DHS,93.3% to DCS,94.1% to PFN and 91.7% to femoral head replacement.[Conclusion]Four kinds of treatment for the aged peoples＇ intertrochanteric fractures have their own advantages and disadvantages.Only taking the patient＇s age,fracture type,osteoporosis and tolerance to the operation into account and paying attention to perioperative care,we can obtain a good clinical effect.
