摘要
[目的]探讨动力髁螺钉(DCS)治疗合并大粗隆游离的股骨粗隆间骨折的临床疗效和特点.[方法]1998年9月～2004年2月应用DCS内固定技术共治疗30例合并大粗隆游离的股骨粗隆间骨折.[结果]随访时间8～25个月(平均16.5个月).复位及内固定满意率为96.7%(29?30),愈合率为100%.并发轻度髋内翻为3.3%(1?30),无下肢外旋畸形.参照韩一生疗效评定标准,髋关节功能优良率为93.3%.[结论]DCS治疗合并大粗隆游离的股骨粗隆间骨折,操作简单,固定牢靠,有利于髋关节功能的恢复,能最大限度减少骨折后的并发症.
[ Objective] To investigate the clinical effects and characteristic of dynamic condylar screw（DCS） for the treatment of femoral i ntertrochanteric fracture with greater trochanter extension. [ Method ] From September 1998 to February 2004,30 patients who suffered from femoral intertrochanteric fracture with greater trochanter extension were treated with DCS. [ Result] The follow-up stage ranged from 8 - 25 months with an average of 16.5 months. In the 30 cases,29 cases（ 96.7 % ） got good reduction and implant position,30 cases（ 100% ） got union,1 case（3.3% ） had mild hipadduation,there was no external rotation deformity of the lower limbs. According to HAN＇ s criteria ,the satisfactory rate of function was 93.3%. [ Conclusion] Applying DCS to treat femoral intertrochanteric fracture with greater trochanter extension, is simple to handle, gives reliable fixation, facilitates rehabilitation of the hip joint ,and can greatly decrease complications of the fracture.
