摘要
目的:探讨Sysmex UF-1000i在老年住院患者尿路感染诊断的特点及应用价值。方法:采集760例老年住院患者尿液标本行UF-1000i尿沉渣分析仪检测及中段尿行细菌培养,统计分析各组尿液白细胞、细菌和类酵母细胞(LYC)计数数据,以细菌培养结果(≥104)为标准,利用ROC曲线,求出各组白细胞、细菌计数对细菌性尿路感染的诊断阈值,并计算其各自灵敏度、特异性。以LYC数值≥4/μl为诊断阈值分析仪器在诊断真菌性尿路感染中应用价值。结果:UF-1000i对老年人细菌性尿路感染白细胞和细菌诊断阈值分别为60.2/μl和185.3/μl,其灵敏度和特异性分别为75%、87.5%和68.1%9、5.2%。对真菌性尿路感染阴性符合率达98.9%,具有较高的筛查价值,随着LYC浓度升高,其阳性符合率也呈上升趋势。结论:UF-1000i尿沉渣分析仪在辅助老年住院患者尿路感染诊断具有较高应用价值。
Objective:To investigate the characteristics and application value of Sysmex UF-1000i in the diagnosis of urinary tract infection in hospitalized elderly patients.Methods: Urine specimens of 760 cases of elderly hospitalized patients were collected for detection by UF-1000i urine sediment analyzer and bacterial culture of the middle urine.The count data of urine white blood cells,bacteria and yeast cell types(LYC) of each group were analyzed in statistics.Taking the results of bacterial culture(≥10^4) as a standard and using ROC curve to get the diagnostic threshold of each group′s white blood cells and bacteria counts on bacterial urinary tract infection and calculate their sensitivity and specificity.LYC values ≥ 4/μl as diagnostic threshold was used to analyze the application value of the instruments in the diagnosis of fungal urinary tract infections.Results: With UF-1000i,diagnostic threshold of white blood cells and bacteria in urinary tract infection of elderly hospitalized patients were 60.2/μl and 185.3/μl respectively.Their sensitivity and specificity were 75%,87.5% and 68.1%,95.2%.The consistent rate to fungal urinary tract infection negative was 98.9% with high screening value.With the increase of LYC concentration,the positive consistent rate rise.Conclusion: UF-1000i urine sediment analyzer has high application value in supporting the diagnosis of urinary tract infection in hospitalized elderly patients.
