摘要
目的调查健康老年人群尿液电导率生物参考值,研究尿液电导率与其他参数的关系及临床应用。方法收集2 806名健康老年人随机尿液,年龄≥60岁,检测尿液电导率和渗透压;各种相关疾病老年患者1 025例,检测尿液电导率和尿钾(K+)、尿钠(Na+)、尿氯(Cl-)、尿素(BUN)、尿肌酐(Cr)、尿尿酸(UA);随机抽取以上尿液标本200例,测定尿比重、尿K+、Na+、Cl-、BUN、Cr、UA。结果健康老年人尿液随机电导率为(17.833±7.989)mS/cm,冰点渗透压为(444.910±151.285)mOsm/kg.H2O。冰点渗透压与电导率之间高度相关(r=0.894,P<0.01)。尿液电导率与渗透压、比重、尿K+、Na+、Cl-、BUN、Cr、UA的相关性分别为0.826、0.520、0.646、0.857、0.857、0.240、0.517、0.474,均呈正相关。结论尿液电导率与渗透压呈正相关,是诊断肾浓缩功能新参数。老年人群尿液电导率低于成年人,应建立自己的生物参考值。
Objective To establish normal reference range of urinary conductivity in healthy elders and study the correlation and clinical application of urinary conductivity and other parameters. Methods Urine samples were taken randomly from 2 806 healthy elders ( age ≥ 60 years) and detected for urinary conductivity and osmotic pressure. 1 025 urine samples from old patients with various diseases were collected and detected the level of conductivity, K^+ , Na^+ , Cl^-, blood urea nitrogen(BUN), creatinine(Cr), uric acid(UA). 200 urine samples were randomly selected from 1 025 patients and detected for the specific gravity, K^+, Na^+, Cl^- , BUN, Cr, UA. Results Urinary conductivity and osmotic pressure in healthy elders were ( 17. 833±7. 989) mS/cm and (444.910±151. 285 ) mOsm/kg·H20, and had high correlation between them (r=0.894,P〈0.01). Conductivity was correlated to osmotic presscere, K^+, Na^+ , Cl^-, BUN, Cr, UA (r=0.826,0.520,0.646,0.857,0.857,0. 240,0.517,0.474). Conclusions The correlation between conductivity and osmotic pressure is good and they are new parameters for renal concentrating function. The urinary conductivity in elders is lower than that in adults, which shows the normal reference range of the urinary condnctivity in elders should be established.
