右美托咪定对七氟烷最低肺泡有效浓度的影响 被引量:11

摘要 目的探讨静脉输注中等剂量右美托咪定(负荷量1 g/kg,维持量0.5 g.kg-1.h-1)对七氟烷最低肺泡有效浓度(MAC)的影响。方法择期上腹部手术患者48例,随机分成对照组(C组,24例)和右美托咪定组(D组,24例)。D组开放静脉后给予右美托咪定负荷量1 g/kg(输注时间10 min),随后以0.5 μg.kg-1.h-1维持;C组静脉输注等量0.9%氯化钠注射液。负荷量输注完毕后采用全凭七氟烷吸入诱导,气管插管成功后行机械通气,使呼气末七氟烷浓度达到预定值并至少稳定15min,开始切皮。呼气末七氟烷预定浓度根据序贯法确定,初始浓度设为2.0%,若体动反应阳性,则下一例患者七氟烷预定浓度增加0.2%;若体动反应阴性,则下一例患者七氟烷预定浓度降低0.2%。将每次切皮时体动反应阴性到阳性的中点定义为一个平衡点,所有平衡点的七氟烷浓度的均数即为MAC,用序贯法公式计算ED50。结果 C组七氟烷MAC为(2.45±0.16)%,ED50为2.43%(95%的可信区间为2.29%~2.57%);D组七氟烷MAC为(2.02±0.14)%,ED50为2.0%(95%的可信区间为1.87%~2.13%);两组比较差异有统计学意义(P<0.01)。结论中等剂量的右美托咪定可降低七氟烷MAC。
