二维斑点追踪技术定量测定左心室容积及整体收缩功能 被引量:3

Quantitative assessment of left ventricular volume and global systolic function with two-dimensional speckle tracking echocardiography

摘要 目的探讨采用二维斑点追踪超声心动图(STE)定量测定左心室容积及左心室整体收缩功能的可行性。方法采用STE在心尖四腔切面测定118人左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、左心室射血分数(LVEF),并与二维超声心动图(2DE)Simpson双平面法及实时三维超声心动图(RT3DE)测定的LVEDV、LVESV和LVEF进行对比及相关性分析。进行STE测量的重复性分析。结果 STE测定的LVEDV与2DE差异无统计学意义,低于RT3DE(P<0.05);STE测定的LVESV与RT3DE差异无统计学意义,高于2DE(P<0.05);STE测定的LVEF低于2DE和RT3DE的测定结果(P<0.001)。STE与2DE及RT3DE测定的LVEDV、LVESV、LVEF具有良好相关性(P均<0.001)。STE测定左心室容积的同一观察者和观察者间变异分别为6.94%、8.31%,测定LVEF的同一观察者和观察者间的变异分别为9.12%、9.87%。结论 STE测定的左心室容积和LVEF与2DE及RT3DE的测定值具有良好的相关性及重复性;STE可用于评价左心室容积及整体收缩功能。 Objective To observe the feasibility of quantitative measurement of left ventricular volume and global systolic function with two-dimensional speckle tracking echocardiography(STE).Methods Data of STE including left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume(LVESV) and left ventricular ejection fraction(LVEF) in the apical four-chamber view were obtained in 118 subjects,and were compared with those of two-dimensional echocardiography(2DE) using Simpson biplane method and real-time three-dimensional echocardiography(RT3DE).The correlation and repeatability among the three techniques were analyzed.Results LVEDV measured with STE and with 2DE were not statistically different,but were significantly lower than that with RT3DE(P0.05).LVESV measured with STE and with RT3DE were not statistically different,but were significantly higher than that with 2DE(P0.05).LVEF measured with STE was significantly lower than that with 2DE and RT3DE(P0.001).Excellent correlation of LVEDV,LVESV and LVEF were noted between STE and 2DE,STE and RT3DE(all P0.001).Intra-and inter-observer variability of STE was 6.94% and 8.31% in left ventricular volume,9.12% and 9.87% in LVEF,respectively.Conclusion Left ventricular volume and LVEF measured with STE have good correlation with those of 2DE and RT3DE,and have more better reproducibility than those of 2DE and RT3DE.STE can be used to assess the left ventricular volume and global systolic function.
