三七总甙抑制鼻内窥镜术后鼻腔粘连形成的研究 被引量:1

摘要 目的:研究三七总甙在用于兔鼻内窥镜术后抑制鼻腔粘连形成的作用及其机制。方法:选择健康成年家兔60只,随机分为3组(实验组:鼻内窥镜术加用三七总甙;空白对照组:鼻内窥镜术;实验对照组:鼻内窥镜术加用丝裂霉素),每组20只。观察各组术后组织病理切片情况及通过透射电镜观察各组细胞的超微结构从而分析三七总甙对鼻粘膜细胞的毒性作用。结果:⑴光镜下组织病理切片观察:实验组、实验对照组术后成纤维细胞数均明显少于空白对照组,差异有显著性(P<0.05);⑵成纤维细胞胞浆中TGF-β1的表达情况:术后第3天,实验组成纤维细胞胞浆中TGF-β1的表达呈弱阳性,空白对照组成纤维细胞TGF-β1的表达呈强阳性。两组比较差异有显著性(P<0.05);⑶透射电镜观察:实验组、空白对照组各细胞的超微结构正常,实验对照组细胞的超微结构破坏。结论:⑴三七总甙在兔鼻内窥镜术后通过抑制成纤维细胞胞浆中TGF-β1的表达减少鼻粘膜瘢痕形成;⑵本实验未发现三七总甙对鼻部组织的毒性反应;⑶三七总甙可作为鼻内窥镜手术的辅助用药。 Objective:To study the anti-conjunctival bleb scarring formation of Panax notoginseng saponins in rabbits after endoscopic sinus surgery.Methods:Endoscopic sinus surgery was performed on 60 noses of 60 healthy adult rabbits which were randomly divided into 3 groups(every group have20 noses of 20 rabbits),experimental group: performed endoscopic sinus surgery using Panax notoginseng saponins.Blank control group: performed endoscopic sinus surgery.Experimental control group: performed endoscopic sinus surgery using Mitomycin.Tissue was harvested from the nasal mucosa area and was made pathological section,then was H-E stained and immunostained and observed under light microscope.TGF-β1 Protein expression and the number of fibroblast were quantitative analyzed by image analysis technique,the toxicity of Panax notoginseng saponins was observed with electron microscope.The data was analyzed statistically by SPSS 12.0 for windows.Results:⑴ Pathological section was observed under light microscope: the number of fibroblast counted by computer showed significant difference between experimental group and blank control group(P0.05),and no obvious difference with experimental control group(P0.05).⑵TGF-β1 expression in the fibroblast endochylema: buffy grains in the fibroblast endochylema under light microscope was considered as positive signal,the intensity of the color reaction in experimental group was weakly positive,while the intensity of the color reaction in blank control group was strongly positive.There was significant difference in the expression of TGF-β1 between experimental group and blank control group(P0.05)at day 3 after surgery.⑶Observed under electron microscope: at day 7 after surgery,the corpuscular ultrastructure in experimental group and blank control group was normal,while the corpuscular ultrastr ucture in experimental control group was destroyed.Conclusion:Experimental obviously inhibited the expression of TGF-β1 of fibroblast and nasal mucosa scarring,Panax notoginseng saponins can be regarded as an adjuvant agent to reduce nasal mucosa scarring after endoscopic sinus surgery.
