摘要
目的分析Wernicke脑病的临床和影像学特点。方法对15例Wernicke脑病的临床表现、MR特点进行回顾性分析。结果 Wernicke脑病的临床症状多样化,其中表现为精神症状12例(80.0%),视力障碍或眼外肌麻痹8例(53.3%),共济失调9例(60.0%)。头颅MR检查,13例(86.7%)出现第三、四脑室及中脑导水管周围、乳头体T1加权低信号,T2加权FLAIR相高信号;治疗后复查明显好转。结论 Wernicke脑病的临床表现无特异性,有发生精神症状、视力障碍或眼球活动异常、共济失调的患者应想到Wernicke脑病的可能性。MR检查可以明确诊断。
Objective To investigate the clinical and imaging characteristics of Wernicke encephalopathy.Methods To analyze the clinical and MRI characteristics of 15 cases with Wernicke encephalopathy and the causes of misdiagnosis in localization diagnosis.Results The clinical characteristics of Wernicke encephalopathy was variety,included psychiatric symptom（80.0%）;visual disorder or ophthalmoplegia externa（53.3%）;ataxia（60.0%）.In cranial MR,13 cases（86. 7%） was found low signals on T1weight（T1W） and FLAIR high signals on T2W in the third and fourth cerebral ventricles, aqueduct of midbrain,bulla of fornix.Conclusion The clinical characteristics of Wernicke encephalopathy had no specificity, based on case history,the patients with pure mental and conscious disturbance and ophthalmoparalys,ataxia had more possibility to suffer from Wernicke encephalopathy.MR examination would help to identify diagnosis.
