摘要
目的:探讨对比增强前后磁共振磁敏感加权成像(susceptibility weighted imaging,SWI)在脑星形细胞肿瘤分级中的应用价值。方法:选取手术病理证实前均行磁共振传统序列及SWI序列扫描的28例星形细胞肿瘤患者,观察肿瘤成像特点并进行比较;对SWI图像ITSS分级,分析其与肿瘤术后病理学分级之间的关系。结果:28例病灶,低级别组10例(Ⅰ级2例,Ⅱ级8例),高级别组18例(Ⅲ级10例,Ⅳ级8例)。18例病灶内显示ITSS,低级别组10例仅2例Ⅱ级肿瘤ITSS分级为1级,余均为0级;高级别组ITSS分级较高,8例多形性胶质母细胞瘤ITSS分级均为3级,间变性星形细胞瘤中2例ITSS分级为0级,3例为1级,3例为2级,2例为3级。ITSS分级和星形细胞肿瘤病理学级别的Spearman相关系数为0.854。结论:SWI序列作为传统序列的有效补充,增强前后SWI图像清晰显示瘤内出血和静脉血管,有助于肿瘤分级的评定,对于指导临床治疗和评估预后具有重要意义。
Objective:To estimate the diagnostic value of pre-and post-contrast-enhanced susceptibility-weighted imaging(SWI) in grading brain astrocytic tumors.Methods:A total of 28 patients(11 women,17 men) with brain astrocytic tumors included in this study.In addition to conventional magnetic resonance imaging,pre-and post-contrast-enhanced SWI were performed in all patients,whose images were analyzed between them.The intratumoral susceptibility signal intensity(ITSS) on SW images was graded,then correlation with the pathological gradation done.Results:To tally 28 tumors were found,and the number of low and high-grade tumors is 10(grade Ⅰ 2,grade Ⅱ8) and 18(grade Ⅲ 10,grade Ⅳ 8).ITSSs were found in 18 tumors,which included 2 low-grade tumors and 16 high-grade tumors.The grade of ITSS of the two low-grade tumors and 8 glioblastomas is 1 and 3,but the grade of ITSS of anaplastic astrocytomas is variable(the number of ITSS scale 0,1,2,3 is 2,3,3,1 respectively).Spearman correlation coefficient between ITSS scale and pathological gradation was 0.854.Conclusion:SWI clearly revealed the intratumoral blood production and small veins of astrocytic tumors,which may be instructive for clinical treatment and prognosis.
