摘要
目的探讨全麻患者麻醉术后下呼吸道感染的相关因素及预防措施。方法将手术患者随机分为两组,均用经气体消毒剂消毒过的麻醉机进行麻醉,Ⅰ组使用一次性过滤器,Ⅱ组不使用;对麻醉机回路内、病毒/细菌过滤器做细菌培养,术后发生下呼吸道感染患者进行痰培养,做常规细菌培养加药敏试验。结果Ⅱ组麻醉机污染率高于Ⅰ组,差异有统计学意义(P<0.01),Ⅰ组术后从麻醉机内和一次性病毒/细菌过滤器采样未捡出细菌,下呼吸道感染率明显低于Ⅱ组。结论气管插管全麻手术患者的医院感染发生率高,应作为医院感染监测重点;针对气管插管全麻患者医院感染的危险因素,提出相应预防措施,降低全麻患者的感染率。
OBJECTIVE To explore the factors and effective preventive method of nosocomial lower respiratory infection after general anesthesia. METHODS Patients were divided into two groups. Both groups were with the gas disinfectant anesthesia machine. In group I , patients were given anesthesia with disposal filter and patients of group II were not, Using bacterial culture and identification technique to detect the samples collected from pipe and the sides of anesthetic machine, from disposable virus/bacteria respiratory filter. The sputum samples of infected patients were taken germiculture and antibiotic susceptibility. RESULTS The rate of infection of anesthetic machine was higher in group II, with significant differences （P〈0.01）. No bacteria growth was found in anesthetic machine and disposable virus/bacteria respiratory filter used after operation in patients without using virus/ bacteris respiratory filter. The incidence of lower respiratory infection was significant lower. CONCLUSION Tracheal intubation of hospital infection of general surgery patients should be as high incidence of hospital infection monitoring key. For patients with trachea general anesthesia hospital infection risk factors, the corresponding prevention measures should be proposed to reduce the rate of general patients trachea.
