摘要
目的探讨大肠癌合并不完全性肠梗阻的手术治疗中结肠镜的使用是否有利于多原发大肠癌的检出。方法对2000年1月～2009年1月南华大学附属第二医院收治的合并不完全性肠梗阻的大肠癌237例的临床资料进行回顾性分析。结果 2005年1月以前128例患者(含2005年1月的病例)术后6个月内复查肠镜发现9例患者距吻合口5cm以上处发现病灶,病理证实为结肠癌,占7.03%;吻合口处病灶并证实为癌者5例,占3.9%;结肠息肉32例,占25%。2005年2月～2009年1月,对109例合并不完全性肠梗阻的大肠癌患者术中行结肠镜检查发现7例合并有多发癌灶,占6.42%;合并大肠息肉21例,占19.2%;6个月内复查发现距吻合口5cm以上发现病灶并证实为癌者1例,占0.91%,吻合口病灶并证实为癌者3例,占2.75%。对比两组患者术后多原发大肠癌的发现情况差异有显著性,P<0.05。结论提高对多原发大肠癌的警惕性,术中仔细探查并结合结肠镜检查有利于多原发大肠癌的检出,并且有利于手术的规范治疗,降低手术后复发几率。
[Objective] To investigate the enteroscope utilization in the course of operation of colorectal cancer with incomplete intestinal obstruction whether or not maked for detection of multiple primary colorectal carcinoma.[Methods] The data of 237 patients with colorectal cancer with incomplete intestinal obstruction from January 2000 to January 2009 were analyzed retrospectively.[Results] The 128 patients from January 2000 to January 2005 were detected with enteroscope within 6 months after operation.The findings were as follows.First,9 patients with colorectal cancer in spinula stoma 5cm accupied 7.03%;Second,5 patients with colorectal cancer in stoma accupied 3.9%;Third,32 patients with colonic polyp accupied 25%.The 109 patients from February 2005 to January 2009 were utilized enteroscope In the course of operation.The finding were that 7 patients with multiple primary colorectal carcinoma accupied 6.42%,21 patients with colonic polyp accupied 19.2%.The 109 patients were detected with enteroscope within 6 months after operation.The findings were as follows;First,1 patients with colorectal cancer in spinula stoma 5 cm accupied 0.91%;second,3 patients with colorectal cancer in stoma accupied 2.75%.[Conclusions] The elevation of vigilance with multiple primary colorectal carcinoma and utilization of enteroscope in the course of operation maked for detection of multiple primary colorectal carcinoma,moreover maked for specification surgical treatment and cutting down the probability of recidivation after operation.
