摘要
目的探讨带锁髓内钉在治疗下肢长管状骨骨折中的应用。方法不同类型下肢长管状骨骨折35例,其中股骨骨折20例、胫骨骨折15例;选用带有远近瞄准器的股骨、胫骨带锁髓内钉治疗。股骨、胫骨横形骨折使用闭合复位穿钉;其它类型股骨、胫骨骨折采用开放复位;粉碎性骨折或多段骨折采用静力固定并一期自体髂骨植骨;术后患肢抬高30,术后第3天开始患肢功能锻炼,对股骨远端及胫骨近端骨折主动锻炼有困难者,术后第3天结合CPM机被动锻炼膝、踝关节。结果随访10月～2年4月(平均13月),35例均骨性愈合,未出现再发骨折、断钉、感染及膝、踝关节僵硬等并发症;2例胫骨远端1/3处骨折出现骨折远端5°向后成角畸形,未出现膝关节、踝关节功能障碍。结论带锁髓内钉治疗治疗下肢长管状骨骨折治愈率高,并发症少。
Objective To explore the efficacy of intramedullary interlocking nail in the treatment of femoral fractures and tibia fractures. Methods Thirty five cases of femoral fractures （n = 20） and tibia factures（ n = 15） were treated with intramedullary interlocking nail with aiming sight between January 2005 and May 2010. Closed reduction was used in transverse femoral and tibia fracture, and open reduction was used in other type femoral fracture. But static force fix was used in comminuted fracture or several section bone fracture combined with autograft bone graft at the same time. The affected limb were put up in a gentle 30 slope after operation. The functional exercised had been carried out from the third day after operation. The passive exercised of both knee and ankle joint had been assisted by CPM machine for those patients with distal femur fracture and proximal tibia fracture if they had difficult in the aspect active exercised. Results The fellow up ranged 10 to 28 months （average 13 months）. All patients had healed up well. There were no complications such as re-fractures, infections and broken nail. But a 5° back angulation abnormity was happened in two patients with distal tibia fracture. Conclusion It can achieve a excellent clinical outcome the use of intramedullary interlocking nail in the treatment of femoral and tibia fractures.
