摘要
目的探讨长柄骨水泥型人工股骨头置换治疗高龄不稳定性股骨转子间骨折的临床疗效。方法对48例〉75岁不稳定性股骨转子间骨折患者采用长柄骨水泥型人工股骨头置换治疗。结果 48例均安全度过围手术期,42例获得随访,时间7~60个月。术后6个月时按Harris评分标准：优24例,良12例,可4例,差2例,优良率达85.7%。随访期内未发现关节松动和下沉。结论高龄不稳定性股骨转子间骨采用长柄骨水泥型人工股骨头置换治疗安全,中近期效果满意,但应严格掌握适应证及加强围手术期处理。
Objective To observe the clinical efficacy and safety for the treatment of unstable intertrochanteric fractures by long stem cemented hemiarthroplasty in elderly patients.Methods 48 cases over 75 years of unstable intertrochanteric fractures were treated with with long stem cemented hemiarthroplasty.Results All cases were safe in perioperative period.42 patients were followed up for 7~60 months.According to Harris′ evaluation standard after 6 months,the results were excellent in 24 cases,good in 12 and fair in 4, and the excellent and good rate was 85.7%.Joint loosening and subsidence was not found during follow-up.Conclusions It is safe and effective for elderly patients to treatment of unstable intertrochanteric fracture of femur with long stem cemented hemiarthroplasty,and short-term follow-up results are satisfactory.The indications should be strictly controlled and perioperative treatment should be enhanced.
