转换酶抑制剂对原发性高血压患者血浆肾素-血管紧张素及心钠素、内皮素的短期影响

摘要 目的 研究血管紧张素转换酶抑制剂(ACEI) 对原发性高血压( EH) 患者肾素 血管紧张素系统(RAS) 、心钠素(ANF) 及内皮素(ET) 的短期影响。方法 口服开搏通50 mg 后,在0 、60 及180 min 时用放射免疫方法测定患者血浆肾素活性(PRA) 、血管紧张素Ⅱ(ATⅡ) 、ANF、ET,同时测定平均动脉压( MAP) 和心率( HR) 。结果 EH 患者的基础ATⅡ、ANF、ET均明显高于对照组( P< 0 .01 或 P< 0 .05) 。服用ACEI 后,PRA 明显升高( P< 0 .01) ,MAP、ATⅡ、ANF 均明显下降( P< 0 .01) ,ET 略有下降( P> 0 .05) 。在MAP 与ATⅡ、ANF、ET 之间无明显相关关系,ATⅡ与ANF 之间也无明显相关性。结论 EH 患者的ANF 增高可能是代偿性的。RAS 及MAP 对ANF 的分泌起着重要作用,ACEI 能通过RAS 影响ANF Objective This study was to understand the acute effects of captopril on atrial natriuretic factor (ANF) and plasma renin activity (PRA),angiotensinII (ATⅡ),endothelin (ET) in patients with essential hypertension (EH). Method PRA,ATⅡ,ANF,ET,was determined by redioimmunoassay at 0,60,180 minutes after giving captopril 50 mg and mean artery pressure (MAP) was measured at same time.Results In EH patients, ATⅡ,ANF and ET were significantly increased ( P <0.01, P <0.05). After administration of captopril, ATⅡ,ANF were significantly decrease ( P <0.01), ET was slightly decrease ( P >0.05). There were no correlations between MAP and ATⅡ or ANF or ET. Conclusion In EH patients, the level of ANF is elevated, the increasing of the lever of ANF may be a compensatory mechanism against hypertension. RAS and MAP play an important role in the secretion of ANF. ACEI can affect the level of ANF through RAS.
