摘要
目的 探讨子宫腺肌病与子宫肌瘤的临床特点及实验室检查的价值，提高子宫腺肌病的术前诊断率。方法 对比分析２５ 例子宫腺肌病患者和３１ 例子宫肌瘤患者的临床资料。结果 两组患者发病年龄及临床表现相近，但子宫腺肌病的痛经症状明显（ Ｐ＜ ０．０５） 。Ｂ 超的诊断符合率子宫肌瘤组和子宫腺肌病组分别为８７．５ ％ 和６０．０ ％ ，两者相比， 有明显差异（ Ｐ＜ ０．０１） 。子宫腺肌病组血清ＣＡ１２５ 水平明显升高， 其阳性率为７２．０ ％ ， 显著高于子宫肌瘤组（阳性率为０）（ Ｐ＜ ０．０１）， 而外周血性激素水平两组比较无统计学差异（ Ｐ ＞ ０．０５） 。结论 子宫腺肌病和子宫肌瘤的鉴别诊断可依靠临床表现及Ｂ 超所见。血清ＣＡ１２５ 水平的测定对提高子宫腺肌病的术前诊断率具有一定价值， 可常规应用，
Objective To evaluate the clinical characteristics and laboratory examination of adenomyosis and hysteromyoma , to improve the accuracy of preoperative diagnosis of adenomyosis.Methods Compare and analyse the clinical informations of 25 adenomyosis and 31 hysteromyomas.Results The ages,symptoms and signs of the patients in the two groups were very similar.But dysmenorrhea was more prominent in adenomyosis, this difference was statistically significant (P<0 05).The accuracy rate of B ultrasonography are 87 5% and 60% to hysteromyomas and adenomyosis respectively.The difference was also statistically significant(P<0 01).The level of serum CA 125 rose evidently , the positive rate was 72%, more prominent than that of hysteromyomas(PR is 0)(P<0 01).There was no statistically significant difference in the level of sexual hormone in the peripheral blood of the two groups (P>0 05) . Conclusion The differentiation of adenomyosis and hysteromyomas is dependent on the clinical symptoms and the imaging of B-ultrasonography.It is of great value to diagnose adenomyosis preoperatively by examining serum CA 125 , it can be used as routine examination, however there is no significance in the examination of peripheral hormone level.
