摘要
目的:探讨螺旋CT对食管癌临床应用价值,并与传统CT相比较。材料与方法:螺旋CT组食管癌12例均行多平面重建,并与35例行传统CT扫描者进行轴位图像比较;7例螺旋CT扫描后在工作站经软件处理行食管仿真内窥镜成像。结果:(1)12例食管癌螺旋CT扫描与传统CT扫描轴位图像所见几乎完全一致,可以观察病变形态,侵及范围,了解有无转移情况等,与手术病理相符。而螺旋CT多平面重建可以沿食管走行进行曲面多平面重建,以最佳角度,不同方位来观察食管肿瘤位置、周围受浸范围。(2)7例食管癌CT仿真内窥镜可从头端或足端入路观察食管壁及肿瘤在腔内的改变等,与纤维食管胃镜所见一致。结论:螺旋CT与传统CT相比,除了可以轴位观察食管病变情况,还可以利用多平面重建及CT仿真内窥镜来观察食管病变特征,为诊断提供更多,更全面的信息。CT信真内窥镜也是纤维食管胃镜的重要补充方法。
Objective: To evaluate the spiral CT
in clinical application for esophageal neoplasm by comparing it with conventiona
l CT.
Materials and Methods:Spiral CT scanning and image process
ing with MPR was performed in 12 cases. A comparison was made between the images
from spiral CT and the images from conventional CT (35 cases). Virtual endoscop
y was conducted on 7 cases scanned by spiral CT, and the findings were compared
with fiberoptic esophagogastroscopy.
Results:(1) On axial images with spiral CT, all lesions of
12 patients showed almost the same pictures as seen with conventional CT. MPR i
n 12 cases could well display esophageal structure through different angle and d
irection. (2) CT virtual endoscopy could let one view esophageal lumen and lesio
n either cephalocaudad or caudocephalad, and the findings were quite consistent
with that seen with fiberoptic esophagogastroscopy.
Conclusion: Compared with conventional CT, spiral CT can n
ot only display esophageal lesions axially, but also can precisely demonstrate t
he morphological features of the lesions with MPR and virtual endoscopy techniqu
e, providing clinicians with more useful information.
