摘要
目的 探讨胫后动脉穿支为蒂的隐神经营养皮瓣的临床应用及皮瓣感觉重建的效果.方法 采用胫后动脉穿支为蒂的隐神经营养皮瓣逆向转位修复胫前及足踝部皮肤软组织缺损17例,并将隐神经与创面近端皮神经进行端端或端侧接合,以重建皮瓣感觉.皮瓣面积:4cm×6cm~9cm×19cm.结果 17例皮瓣全部成活,随访3~24个月.皮瓣感觉恢复S2 6例,S3 8例,S3+3例.结论 胫后动脉穿支为蒂的隐神经营养皮辩可较好地修复胫前及足踝部皮肤缺损,神经修复后的感觉恢复也满意.
Objective To study use of the transfer of perforator sural the posterior tibial artery (PTA) reversed saphenous neurocutaneous flap based on the with sensory reconstruction. Methods Flap transfer for the perform coverage of the reconstruction of the extensive defect in the lower extremity in 17 patients. The size of the flaps ranged from 4cm ×6cm to 9cm×19cm Thereafter, neurorrhaphy was performed between the saphenous nerve in the flap and other cutaneous nerve in this region in an end- to-side or end-to-end fashion.Results In this series ,flaps survived. At the final follow up of 3 to 24 months, flap sensation was S2 in 6, S3 in 8, and S3 + in 3 cases. Conclusion The reversed saphenous neurocutaneous flap based on the perforator of the PTA ’s useful for coverage in the lower extremity with sensory reconstruction.
