摘要
目的探讨煤工尘肺患者合并左心衰竭的发生率及发病因素。方法收集门矿医院3年住院的全部煤工尘肺患者临床资料,对发生左心衰竭的患者的基本特征和诱发原因进行描述性分析。结果煤工尘肺患者左心衰竭发生率14.09%,煤工尘肺不同期别发生率无显著性差异。患者均继发慢性阻塞性肺疾病(COPD),肺心病患病率83.87%、高血压70.97%、冠心病61.29%、脑梗塞51.61%。结论煤工尘肺合并左心衰竭发生率高于一般老年人群,左心衰竭的发生率与尘肺期别无关,煤工尘肺患者并发肺心病、高血压、冠心病易发生左心衰,另外部分病人罹患脑血管病反应性差和表达不清也是左心衰竭加重的重要原因。
Objective To explorer incidence and risk factors on coal worker＇s pneumoconiosis combined with left-sided heart failure.Methods Collection all the clinical data of coal worker＇s pneumoconiosis inpatient in Menkuang hospital 3 years,find the patients who combined with left-sided heart failure and get them general characters and precipitating factors by descriptive analysis.Results Left heart failure incidence in coal worker＇s pneumoconiosis is 14.09%,the incidence of pneumoconiosis in various stages have no significant differences.All the patients secondary COPD,incidence of pulmonary heart disease is 83.87%,hypertension is 70.97%,coronary heart disease is 61.29%,cerebral infarction is 51.61%.The main precipitating factors are lung infection and infusion.Conclusion The incidence of coal worker＇s pneumoconiosis combined with left-sided heart failure is higher than general elder crowd,and the incidence of left heart failure isn＇t related with the various stages of pneumoconiosis.The cause of acute left heart failure is complicated,and often the joint action of many diseases,pulmonary heart disease, hypertension,coronary heart disease is the main reason,while some patients suffering from cerebrovascular disease,poor response and expression is also important reasons lead to left heart failure.
