摘要
目的探讨老年患者截石位术后双下肢不同放法对血流动力学的影响作用。方法将172例采用膀胱截石位手术的老年患者随机分为观察组88例和对照组84例,观察组术中每隔1h抬高患者小腿一次,每次10min,以防下肢深静脉血栓形成,术毕由截石位转变为平卧位时,先缓慢平放一侧下肢,轻拍小腿肌肉,做被动屈膝运动,3min后再放另一侧下肢。对照组患者则同时平放双下肢,分别观察记录两组患者术后双下肢平放前、后血压变化的情况以及术后下肢深静脉血栓形成的情况,并进行对照比较。结果观察组患者术后下肢深静脉血栓形成占5.68%,而对照组患者术后下肢深静脉血栓形成占22.62%,两组比较,差异有统计学意义(p<0.01);观察组患者双下肢平放前、后血压下降的程度较小,对照组患者血压下降的程度较大,两组比较,差异有统计学意义(p<0.01)。结论膀胱截石位手术的老年患者,术中每隔1h抬高小腿一次,有效预防老年患者术后下肢深静脉血栓形成,术后由截石位转变为平卧位时,采用下肢单腿缓慢平放法可使血压变化平稳,对减少术后并发症、促进术后康复具有非常重要的现实意义,值得临床进一步推广。
Objective To explore the affect of lower limb different placement of elderly patients after lithotomy position operation on blood flow dynamics.Methods 172 elderly patients undergoing lithotomy position operation are randomly divided into 88 cases as the research group and 84 cases as the control group.The lower limbs of patients of the research group were lifted every 1 hour for 10 minutes to prevent from the formation of the deep vein thrombus.After the operation,turning the lithotomy position to horizontally lay position lay down one side lower limb slowly,clap the muscle,make passive kneebend for 3 minutes,then lay down the other side lower limb for 3 minutes.In control group the two lower limbs were lay down simultaneously.Observe and record separately the blood pressure before and after lay down the lower limbs in the two groups,and the condition of formation of deep vein thrombus,then compare the results of two groups.Results The formation of the deep vein thrombus of lower limbs of the patients of the research group was 5.68% after the operation but the formation of the deep vein thrombus of lower limbs of the control group was 22.62% after the operation.The comparison of two groups has different statistic meaning（p0.01）.Before and after the lower limbs lay down,the blood pressure dropping in research group is less than that in control group.The comparison of two groups has different statistic meaning（p0.01）.Conclusion In elderly patients undergoing lithotomy position operation,to lift the lower limb every one hour can effectively prevent the formation of deep vein thrombus,while adopting slow lay down the lower limbs during turning lithotomy position to horizontally lie on can smooth the blood pressure changes.All these measures have very important real significance to reduce postoperative complication and promote postoperative rehabilitation,so are worthy to generalized in clinic.
