摘要
目的探讨输尿管镜下双频双脉冲掺钕钇铝石榴石(Nd:YAG)激光(U100激光)治疗输尿管结石的疗效。方法应用U100激光经输尿管镜治疗输尿管结石409例(患者共378例,双侧者31例,共409侧。本文按409例计)。结石位于上段91例,中段243例,下段75例。术前192例行SWL治疗。结果碎石成功率为95.8%(392/409),上段为91.2%(83/91),中段为97.1%(236/243),下段为97.3%(73/75)。其中5例因结石坚硬,改为气压弹道碎石;12例有大块结石移入肾盂,留置D-J管后行SWL。术后住院时间2～5d,平均3d;平均激光工作时间(5.5±3.5)min。无大出血、输尿管撕脱及脓肾等严重并发症。结论输尿管镜下U100激光碎石具有快速、安全、微创、碎石成功率高和患者恢复快等优点,是一种治疗输尿管结石的理想方法。
Objective To evaluate the curative effect of frequency-doubled double-pulse Nd？？YAG laser in treatment of ureteral calculus.Methods Totally 409 cases with ureteral calculus treated via endoscope with U100 laser were summarized.Results The successful rate was 91.2% （83/91） in the proximal ureteral calculus,97.1% （236/243） in the middle ureteral calculus,97.3% （73/75） in the distal ureteral calculus and 95.8% （392/409） in all.While the other 17 patients still suffered because of the remained stone fragments,among whom 12 cases of stone migration occurred and 5 cases of stones were too impacted.Hospitalization,on average,was 3 days with a range of 2～5 days.The laser worked （5.5-3.5）minutes in average.No major complications such as hemorrhoea,high fever or pyonephrosis were observed.Conclusions The frequency-doubled double-pulse Nd：YAG laser used with ureterorenoscopy for endoscopic lithotripsy is an effective and harmless method with high success ratio,after which patients recover quickly.It is an ideal therapeutic tool for ureteral stones.
