摘要
心脏再同步化治疗(CRT)能够改善心力衰竭(简称心衰)患者的预后,而接受CRT的患者大约仍有30%对CRT反应差,而且具有较高的死亡率。研究显示心衰恶化(也称泵衰竭)和致命性心律失常引起的心源性猝死是CRT术后患者两大主要死亡原因;高龄、男性、右束支传导阻滞、缺血性心肌病、术前心衰的严重程度、合并有高血压、糖尿病、肾功能不全、阵发性心房颤动等病史以及术后3~6个月未发生心室重构逆转等是CRT死亡的影响和预测因素。对这些因素进行及时的评估和有效的干预,可能会提高CRT的疗效,进一步降低患者死亡率。
