慢性心力衰竭患者贫血与肾功能不全的相关性研究 被引量：13

摘要 目的:探讨慢性心力衰竭(CHF)患者贫血与肾功能的相关性。方法:对入选的156例CHF患者测定血红蛋白(Hb)、肾功能尿素氮(BUN)、肌酐(Cr)、超声心动图测量左室射血分数(LVEF);按Hb水平分为贫血组(57例)和非贫血组(99例)比较贫血组与非贫血组肾功能不全发生率;对Hb、LVEF、BUN、Cr做相关关系分析。结果:贫血发生率为36.5%,肾功能不全发生率为21.7%,随NYHA递增,贫血和肾功能不全逐渐增加,Hb、EF水平下降,BUN、Cr水平递增(P<0.05);贫血组患者肾功能不全发生率水平明显高于非贫血组(P<0.01);相关关系分析显示Hb与LVEF呈正相关(r=0.321,P=0.018),与尿素、Cr呈负相关(r=-0.418,P=0.000;r=-0.676,P=0.002)。LVEF与BUN、Cr呈负相关(r=-0.216,P=0.008,r=-0.310,P=0.012)。结论:CHF患者随心衰加重,贫血和肾功能不全发生率增加,贫血及肾功能不全与心力衰竭病情加重相关。
