CT检查预测尿石成份及体外碎石效果的研究 被引量:15

摘要 目的探讨尿结石CT值和结石成份间的关系及评价体外冲击波碎石(ESWL)效果。方法收集197例经ESWL治疗的尿结石患者,术前行CT扫描测定结石软组织窗CT值,术后对病人行B超或腹平(KUB)检查,观察碎石形状变化,收集碎石屑测定化学成份,统计各种结石的对应CT值范围;根据碎石屑残留情况分排空组及残余组,统计学分析两组间结石CT值与排空率差异。结果软组织窗下纯尿结石的CT值为草酸钙(990~1291)Hu、磷酸钙(925~1146)Hu、胱氨酸(708~899)Hu、磷酸镁铵(553~625)Hu和尿酸(369~483)Hu,混和尿结石的CT值均在相应纯尿结石成份的CT值之间;排空组结石的CT值为629.39±199.18Hu;残余组结石的CT值为1027.02±205.62Hu,差异有统计学意义(P<0.01),显示结石CT值与排空率呈反相关关系。结论平扫结石CT值能预测尿结石的主要成份,可以作为评估ESWL治疗效果参考指标之一。 Objective To explore the relationship between urinary calculi CT value and calculus composition,and essess treatment effect on extracorporeal shock wave lithotripsy(ESWL).Methods 197 patients with urinary calculi by ESWL treatment were collected,who were performed a preoperative CT scan to determine the CT value of calculi under soft tissue window and who were conducted immediate postoperative b-sonography or abdominal plain sheet(KUB) check,observing the gravel shape,collecting and determining the chemical composition of the gravel crumbs,measuring the statistic corresponding CT value scope of different types of calculus;According to the gravel crumbs'residue,they were divided into emptying group and residual group to perform statistical analysis between the two groups about the stone CT value and different rates of stones emptying.Results The CT value of pure urine calculus under soft tissue window was listed as calcium oxalate(990-1291) Hu,calcium phosphate(925-1146) Hu and cystine(708-899) Hu,phosphate magnesium ammonium(1999-625) Hu and uric acid(369-483) Hu.Meanwhile,The mixed Urinary calculi CT value exists in the corresponding value of pure urinary calculi composition;The average stone CT value for the emptying groups showed as(629.39±199.18)Hu;The average stone CT value for the residual groups as 1027.02±205.62 HU;The difference was statistically significant(P0.01),Cindicting negative correlation between calculi CT value and rates of stones emptying.Conclusion The consequences from plain CT scan can predict the main components of the urinary calculi and can be used as a referential evaluation indexes to ESWL therapeutic effect.
