多药耐药鲍曼不动杆菌β-内酰胺酶基因和膜孔蛋白基因研究 被引量:7

摘要 目的调查多药耐药鲍曼不动杆菌(MDR-ABA)β-内酰胺酶基因和膜孔蛋白carO基因情况。方法收集分离自宁波市李惠利医院2009年6月至2009年9月住院患者的MDR-ABA共20株,采用聚合酶联反应(PCR)及序列分析的方法分析21种β-内酰胺酶基因和膜孔蛋白carO基因。结果 20株MDR-ABA共检出3种β-内酰胺酶基因:TEM、SHV、ADC,阳性率分别为80.0%(16/20)、5.0%(1/20)、30.0%(6/20),其余18种β-内酰胺酶基因未检出,膜孔蛋白carO基因缺失率70.0%(14/20)。2号株ADC阳性基因测得序列经BLAST比对与已在GenBank登录的ADC型AmpC均不相同,经与GenBank登录的ADC型序列分子进化分析,确认为ADC型β-内酰胺酶新的变异型。结论本组MDR-ABA多种β-内酰胺类药物耐药与产TEM、SHV、ADC等3种β-内酰胺酶相关外,还与膜孔蛋白编码基因carO缺失有关。 Objective To investigate the distribution of β-Lactamase genes and porin gene (carO) in multidrugresistant Acinetobacter baumannii (MDR-ABA). Methods From Jun 2009 to Sep 2009, 20 strains of MDR-ABA were collected from Ningbo Medical Treatment Center Lihuili Hospital. Then, 21 kinds of β-Lactamase genes and porin gene (carO) were analyzed by PCR and verified by DNA sequencing. Results In 20 strains of MDR-ABA, 3 kinds of β-lactamase genes (TEM, SHV, ADC) were detected out,the positive rates of TEM, SHV, ADC in 20 strains of MDR-ABA were 80.0%, 5.0% and 30.0%. However, the other 18 kinds of β-Lactamase genes couldn't be detected out. Loss ofPorin gene (carO) was identified in 14 (70.0%) of 20 strains of MDR-ABA. Sequence ofADC β-Lactamase gene in strain No.2 compared with GenBank logged in AmpC ADC was not the same type by BLAST, and identified as a new variation type ofADC β-lactamases genes by phylogenetic analysis. Conclusion Besides TEM, SHV, ADC β-Lactamase, it has been shown that resistance to β-Lactams in this group of MDR-ABA is associated with the loss of porin gene (CarO).
