摘要
目的报道游离足底内侧及足内侧双叶皮瓣修复手部皮肤缺损的手术方法及临床效果。方法在解剖研究的基础上设计以足底内侧血管为主干，带其主干支及足内侧区皮穿支构成足底内侧非负重区、足内侧区双叶皮瓣，游离移植同时修复手或手指两处皮肤缺损。结果临床应用５例，皮瓣全部成活，随访６～２４个月，外形与功能改善满意。结论足底内侧区双叶皮瓣游离移植的成功为手部多处皮肤缺损的修复提供了一种较理想的术式。
Objective To report the operative technique and clinical results of using a free bifoliated flap combining instep flap and medialis pedis flap to repair the skin defects of the hand Method Based on the anatomical studies,a bifoliated flap composed of instep and medialis pedis flap was designed,the combined flap contained medial plantar artery and the proximal or middle cutaneous branch of the medial plantar artery The free bifoliated flap were used to cover the two site skin defects of the hand and fingers simultaneously Results The flap was applied clinically in 5 cases,all of skin flaps were survived completely The outline of the hand and functional results were satisfactory after 6～24 months follow up Conclusion The free bifoliated flap combining instep and medialis pedis flaps provided a new ideal option for the repairing of mutiple skin defects of the hand
