乌拉地尔和一氧化氮对急性缺氧犬肺循环的作用

摘要 目的 :对比观察乌拉地尔 (URA)和一氧化氮 (NO)对肺循环和体循环的影响及作用的强弱。方法 :健康杂种犬 8只 ,先观察静注URA 1mg/kg后 1~ 3 0分钟血流动力学的变化 ,然后使FIO2 降至 0 12 ,按随机顺序进行两组实验。组 1静注URA 1mg/kg ,5分钟后再吸入 90× 10 -6 容积的NO ;组 2先吸入 90× 10 -6 容积的NO ,再静注URA1mg/kg ,观察用药后血流动力学变化。结果 :静注乌拉地尔可同时降低正常犬体循环和肺循环的阻力及压力 (P <0 0 5 ) ,作用约持续 10分钟。组 1静注URA可明显降低缺氧犬MAP、SVR(P <0 0 5 ) ,再吸入NO后可再降低MPAP、PVR和SVR(P <0 0 5 ) ;组 2吸入NO后缺氧犬的PVR显著降低 (P <0 0 5 ) ,再静注URA后 ,MPAP、MAP及SVR又明显下降 (P <0 0 5 )。结论 :在急性缺氧犬模型上 ,URA静注后的扩血管作用以体循环为主。 Objective: To observe and compare the effect of urapidil and nitric oxide (NO) on pulmonary and systemic circulation. Methods: Eight healthy dogs were studied. At first, the hemodynamic parameters were monitored at 1~30 min after intravenous administration of urapidil 1mg/kg, then the F IO 2 was decreased to 0 12, and two experiments were carried out. Experiment 1: 1mg/kg of urapidil was administred intravenously, followed by inhalation of 90×10 -6 NO 5 min later. Experiment 2: 90×10 -6 NO was inhaled, followed by intravenous administraion of 1mg/kg urapidil. The hemodynamic changes were observed in both experiments. Results: Intravenous adminisration of urapidil could decrease the resistances and pressures in both systemic and pulmonary circulation in normal dogs (P<0 05). The effects lasted for 10 min. In experiment 1 intravenous administration of urapidil could significantly decrease mean arterrial pressure (MAP) and systemic vascular resistance (SVR) (P<0 05) in hypoxic dogs. NO inhalation could further decrease MPAP, pulmonary vascular resistance (PVR) and SVR (P<0 05). In experiment 2 NO inhalation decreased PVR (P<0 05) in hypoxic dogs. The administration of urapidil could further obviously decrease MPAP, MAP and SVR (P<0 05). Conclusion: The vessel dilating effect of intravenous urapidil is prominent on systemic circulation in acute hypoxic dogs.
