摘要
目的探讨Rockall危险性积分评估上消化道再出血和死亡风险的临床价值。方法根据Rockall危险性积分将首次发生上消化道出血且已行急诊内镜治疗的118例患者分为高危组、中危组和低危组,同时针对各组实施不同的护理干预措施后,比较各组间再出血率和死亡率差异。结果高危组患者再出血率显著高于低危组(χ2=8.801,P=0.003),中危组再出血率亦明显高于低危组(χ2=6.789,P=0.009),但高危组和中危组之间再出血率无统计学差异(χ2=0.497,P=0.481);高危组和中危组死亡率明显高于低危组(χ2=15.523,P<0.0001;χ2=6.545,P=0.011),高危组死亡率亦明显高于中危组(χ2=4.636,P=0.031)。结论 Rockall危险性积分是评估上消化道再出血和死亡风险的有效工具,它对临床诊治和护理具有重要指导意义。
Objective To investigate the clinical value of Rockall risk scoring for upper gastrointestinal re-bleeding and death risk. Methods One hundred and eighty patients who suffered from first onset of upper gastrointestinal bleeding and underwent emergency endoscopic treatment were divided into high-risk group,risk group and low-risk group according to Rockall risk scoring.After given different nursing interventions according to the conditions,the re-bleeding and mortality rates of each groups were compared. Results The re-bleeding rate of patients in the high-risk group was significantly higher than that in the low-risk group（χ2=8.801,P=0.003）,and the re-bleeding rate of patients in the moderate-risk group was also significantly higher than that in the low-risk group（χ2=6.789,P=0.009）.However,there was no significant difference re-bleeding rate between high-risk group and moderate-risk group（χ2=0.497,P=0.481）.In addition,the mortality rate of patients in the high-risk and moderate-risk groups was significantly higher than that in the low-risk group（χ2=15.523,P0.0001;χ2=6.545,P=0.011）,and the mortality rate of patients in the high-risk group also higher than those in the moderate-risk group（χ2=4.636,P=0.031）. Conclusion Rockall risk scoring is an effective tool to assess the risk of gastrointestinal re-bleeding and death risk,and it plays an important role in clinical diagnosis and treatment as well as nursing care.
