摘要
目的探讨神经降压素(neurotensin,NT)在妊高征(PIH)中的作用及其与PIH的发生、发展的关系。方法对妊娠中期用平均动脉压预测PIH呈阳性者(MAP≥11.3KPa)20例及轻、中、重度PIH各24例为实验组,预测呈阴性者20例及正常足月孕妇24例为对照组,采用放射免疫法(RIA),检测血浆的NT水平。结果妊娠中期预测试验阳性者NT水平为90.45±15.00pmol/L,阴性者为113.33±13.47pmol/L,阳性者NT水平显著低于阴性者(P<0.01)。轻度PIH及正常足月孕妇产前NT水平分别为100.56±26.35pmol/L、104.17±29.66pmol/L,产后3天NT水平分别112.35±25.55pmol/L、110.23±30.52pmol/L;中度及重度PIH产前NT水平为84.56±30.99pmol/L及76.02±20.85pmol/L,产后3天NT水平为105.78±28.63pmol/L及98.06±24.10pmol/L。中、重度PIHNT水平明显低于轻度PIH及正常足月孕妇(P<0.05),其产前NT水平也明显低于产后(P<0.05)。结论NT水平的降低与PIH的发生有关,其降低程度与PIH的病情相关;连续监测NT的变化,有可能对PIH的预测有一定意义。
To study the effect of neurotensin (NT) in pregnancy induced hypertension (PIH) patients and the relation between NT and the cause and the development of PIH.Metheds The NT level in plasma of 20 mid-term pregnant patients whose PIH prediction test waspositivte (mean artery pressure MAP≥11. 3 Kpa) and 24 patients of mild, moderate and severe PIHwas determined by radioimmunoassay (RIA). Twenty patients whose PIH prediction test was negative and 24 normal full-term pregnant patients werc set as control. Results The NT level in themid-term pregnant patients with a positive PIH prediction test was 90. 45±15. 00pmol/L, and theNT level in mid-term pregnant patients with a negative PIH prediction test was 113. 33±13. 47 pmol/L. The positive team was significantly lower than the negative team (P<0. 01)- The NT levels ofmild PIH and normal full-term pregnant patients were 100. 56±26. 35 pmol/L and 104. 17±29. 66pmol/L before labor and 112. 35±25. 55 pmol/L and 110. 23 ±30. 52 pmol/L 3 days postpartumwhile the NT levels of moderate and severe PIH were 84. 56 ± 30. 99 pmol/L and 76. 02 ± 20. 85pmol/L before labor and 105.78±28±28. 63 pmol/L and 98.06±24.10 pmol/L 3 days postpartum. TheNT levels in the moderate and severe PIH team was significantly lower than those in the mild PIHand normal full-term pregnant patients (P<0. 05). In the moderate and severe PIH patients, the NTlevel before labor was significantly lower than postpartum patients, too (P<0. 05). ConclusionThe decreased NT level is correlated to the cause of PIH, and the degree of decreasing of the NT level is related to the severity of PIH; continuous monitoring of the NT level may have some significancein predicting PIH.
