摘要
目的:评价应用微型外固定架和AO微型钢板内固定治疗掌指骨骨折的疗效。方法:2009年1月~2010年1月对38例49处手掌指骨骨折分别应用微型外固定架和AO微型钢板治疗,并对两组分别进行TAFS评分。结果:所有病例均达骨性愈合。AO微型钢板组TAFS评分中,优良率为78.4%,而外固定架组为84.9%。两者的TAFS功能评分结果差异有统计学意义(P<0.05),后者明显优于前者。AO微型钢板组延迟愈合率为2.70%;外固定架组为1.84%,后者优于前者。AO微型钢板组平均骨折愈合时间为4.5周,外固定架组为3.6周,两者结果差异有统计学意义(P<0.05),前者患者骨折愈合时间明显长于后者。两者术后感染率相似。结论:微型外固定架治疗手部开放性粉碎性掌指骨骨折创伤小,固定可靠,愈合率高,利于早期功能锻炼,有良好的治疗效果,较传统AO解剖钢板内固定疗效更好。
Objective: To evaluate the micro-mini-external fixator and AO plate fixation of metacarpal and phalangeal open comminuted fracture.Methods: From 38 cases since January 2009 to January 2010,49 hand open comminuted metacarpal and phalangeal fractures were AO mini external fixator and the micro-plate for treatment,and two separate TAFS score.Results: All patients reached the bone healing.TAFS score AO mini-plate group,the excellent rate of 78.4% while the external fixation group was 84.9%.The results of both TAFS function score was significantly (P0.05),which was better than the former.AO mini-plate group delayed union rate of 2.70%;external fixation group was 1.84%,the latter than the former.AO mini-plate group,the average healing time was 4.5 weeks,the external fixation group was 3.6 weeks,there was significant difference between the two results (P0.05),the former cases,fracture healing time was significantly longer than the latter.Postoperative infection rates between the two similar.Conclusion: Mini external fixation of open comminuted hand metacarpal and phalangeal fractures with small,fixed and reliable,high healing rate,conducive to early exercise,have good therapeutic effect than conventional plate fixation of AO anatomy better.
