解剖型钢板及骨诱导活性材料治疗塌陷性跟骨骨折 被引量:2

Treatment for Compressed Calcaneus Fracture with Anatomic Plate Internal Fixation and Osteoinduction Active Material

摘要 目的:评估切开复位跟骨解剖型钛板内固定及骨诱导活性材料填塞治疗跟骨塌陷性骨折的临床疗效。方法:32例单侧跟骨塌陷性骨折采用跟骨外侧入路切开复位、解剖型钛板螺钉内固定、骨缺失处填入骨诱导活性材料治疗。术后18个月时对随访到的31例进行评估。结果:B?hler角术前0°~12°,平均7.6°;术后18个月时32°~40°,平均34.2°。Gissane角术前75°~90°,平均87°;术后18个月时115°~135°,平均126°。手术前后比较,差异性显著(P<0.05)。AOFAS评分75~100分,平均90.5分;优良率87.6%。结论:跟骨塌陷性骨折切开复位后行钢板内固定及骨诱导材料植入,能有效保持跟骨及其关节面的解剖形态,且足部功能恢复较好;骨诱导材料具有良好的组织相容性,是治疗跟骨塌陷性骨折的一种较好方法。 Objective To evaluate the effect of treatment for compressed calcaneus fracture with anatomic plate internal fixation and osteoinduction active material. Methods Thirty-two patients with compressed calcaneus fracture were treated with anatomic plate titanium internal fixation combined with cancellous bone screw and osteoinduction active material grafting. After 18-month follow-up, evaluation of the 31 patients was carried out. Results The preoperative Bohler’s angle ranged from 0 to 12°, 7.6°on average, and postoperative from 32°to 40°, 34.2°on average. The preoperative Gissare’s angle was 75°to 90°, 87°on average; and postoperative from 115°to 135°,126°on average. There was statistic difference (P 0.05). AOFAS score was from 75 to 100 pints, 90.5 points on average. The excellent and good rate was 87.6%. Conclusion The treatment of compressed calcaneus fracture with anatomic plate internal fixation and osteoinduction active material has the advantages of maintaining the anatomy form, recovering function and having good histocompatability. It is an excellent therapy for compressed calcaneus fracture.
