摘要
目的:评价膜式液基薄层细胞学技术(TCT)、高危型人乳头状瘤病毒(HR-HPV)检测和阴道镜下宫颈活检对子宫颈病变的临床诊断价值。方法:320例TCT检查异常的患者中,160例应用PCR荧光技术检测HR-HPV,186例进行阴道镜检查并取宫颈组织活检,136例同时行HR-HPV检测与阴道镜检查并取宫颈组织活检。以病理学作为诊断标准,比较3种方法对宫颈病变的诊断价值。结果:HR-HPV检测的阳性率56.9%;宫颈活检者病理学诊断为上皮内病变者135例,占72.6%;同时行HR-HPV检测与电子阴道镜宫颈活检的患者中,84例HR-HPV检测阳性者中CIN占32.1%,HR-HPV检测阴性者中CIN占13.5%(P<0.05)。结论:TCT、HR-HPV检测与阴道镜下宫颈活检均是子宫颈病变早期诊断的检测方法,三者各有优缺点,不能相互替代,三者结合能提高宫颈病变的检出率。
Objective：To evaluate the clinical values of ThinPrep cytology test（TCT）,high risk human papillomavirus（HPV） detection and cervical biopsy under colposcope in diagnosis of cervical lesions.Methods：Among 320 cases with abnormal TCT results,160 cases received high risk HPV detection by PCR fluorescence technique,186 cases received colposcopy and cervical biopsy,136 cases received high risk HPV detection,colposcopy and cervical biopsy;pathological diagnosis was gold standard;the values of the three methods in diagnosis of cervical lesions were compared.Results：The positive rate of high risk HPV detection was 56.9%;135 cases were diagnosed as intraepithelial lesions by pathological examination of cervical biopsy,accounting for 72.6%;among the cases receiving high risk HPV detection,colposcopy and cervical biopsy,the proportions of CIN among high risk HPV positive cases and high risk HPV negative cases were 32.1% and 13.5%,respectively,there was significant difference（P〈0.05）.Conclusion：TCT,high risk HPV detection and cervical biopsy under colposcope are early diagnostic methods of cervical lesions,the three methods have advantages and disadvantages,and they are not interchangeable,joint detection of the three methods may improve the detection rate of cervical lesions.
