摘要
目的探讨HBV-DNA,荧光定量,乙型病毒性肝炎(简称乙肝)前S1抗原(PreS1)与HBV血清标志物(HBVm)之间的相关性。方法采用荧光定量PCR技术,酶联免疫吸附法(ELISA)对200份HBsAg阳性样本进行HBV-DNA,PreS1,HBVm检测。结果在HBV-DNA荧光定量阳性的158份血清样本中(病毒载量>103拷贝/mL)PreS1抗原阳性率达到81.6%(129/158),并且血浆中HBV-DNA的含量水平与PreS1抗原阳性检出率间存在显著相关性(P<0.001)。HBsAg阴性109份,HBV-DNA荧光定量阳性率为64.2%;PreS1抗原阳性率为48.6%。急性肝炎中HBV-DNA及PreS1的阳性检出率分别为96%和92%,在轻度慢乙肝中,HBV-DNA及PreS1的阳性检出率分别为83.3%和70.0%。在此两类HBV-DNA与PreS1的阳性检出率没有显著性差异(P>0.05)。而在中重度慢性乙肝及肝炎肝硬化标本中HBV-DNA的阳性检出率明显高于PreS1(P=0.01)。结论 PreS1可作为HBV-DNA荧光定量的有益补充用来判断病毒复制情况,对乙型肝炎诊断、疗效判断具有十分重要的意义。
Objective To study the correlation between HBV-DNA PreS1 antigen and serum markers of HBV.Methods PreS1 antigen,serum markers and HBV-DNA viral loads of 200 Sera of patients with HBsAg were detected by ELISA（enzyme linked immunosorbent assay）and FQ-PCR.Results The positive rate of PreS1 antigen was 81.6%（129/158）among 158 patients with detectable HBV-DNA viral loads（103 copies/mL）,and there was a significant correlation between HBV-DNA viral loads and the positive rates of PreS1 antigen（P0.001）.In HBeAg negative 109 samples,the positive rates of HBV-DNA and PreS1 antigen were 64.2% and 48.6%,respectively.In samples with acute HBV infection,the positive rates of HBV-DNA and PreS1 antigen were 96% and 92%,respectively.In samples with mild chronic HBV infection,the positive rates of HBV-DNA and PreS1 antigen were 83.3% and 70%,respectively.There were no significant difference between the positive rates of HBV-DNA and PreS1 antigen in there two groups（P0.05）.But the rate of HBV-DNA was significantly higher than the positive rate of PreS1 antigen in samples with medium and heavy chronic HBV infection and cirrhosis（P=0.01）.Conclusion The detection of PreS1 antigen could be used as supplement for HBV-DNA to evaluate the situation of HBV replication.which is very important for the diagnosis of HBV infection and the evaluation of treatment.
