摘要
目的:分析小儿先天性心脏病术后感染的菌群及细菌耐药性,为临床抗菌药物的使用提供参考。方法:回顾性分析2008年9月～2009年5月48例先天性心脏病手术后感染患儿资料,记录感染者细菌培养及细菌药敏实验结果,并进行分析。结果:48例患儿的痰液、血液、导管分泌物中分离80株感染菌,其中革兰阴性菌53株(66.25%);革兰阳性菌15株(18.75%);类酵母菌12株(15.00%)。多数细菌对美罗培南、头孢哌酮/舒巴坦、阿米卡星及亚胺培南敏感。预防感染使用抗菌药前两种分别为头孢替安(74.03%)和头孢曲松(19.06%);感染控制使用抗菌药前三种为美罗培南(29.60%)、头孢哌酮/舒巴坦(24.80%)及氟康唑(20.00%);抗菌药单用8例,其余40例联合使用两种或两种以上抗菌药。结论:小儿先天性心脏病术后感染应该根据病原学及药敏结果合理用药。
Objective ：To test the post-surgery pathogens for patients suffering from pediatric cardiac disease, to examine the drug resistance of pathogens and provide reference for the clinical use of antibiotics. Method ：48 cases infected children after cardiac surgery from August 2008 to May 2009 were analyzed, recorded the experimental results concerning the cultivation and drug resistance of pathogens. Result： From the patients＇ phlegm, blood, and secretion, we had separated 80 pathogens, and among which there were 53 Gram-negative bacteria, accounting for 66.25% of the total; 15 Grampositive bacteria, 18.75% of the total; and 12 fungi, 15.00%. Most bacteria were responsive to Meropenem, Cefoperazone and Sulbactam,Amikacin, and Imipenem. The pre-surgery antibiotics we used were Cefotiam （74.03%） , and Ceftriaxone （ 19.06% ） ; the antibiotics we used to control post-surgery infection were Meropenem （29.60%）, Cefoperazone and Sulbactam （24.80%）, and Fluconazole （20.00%）. Among the 48 patients, 8 patients Were given a single antibiotic, and the other 40 ones were given 2 kinds or more than 2 kinds of antibiotics at the same time or separately. Conclusion： The post-surgery pediatric cardiac patients should be given proper medicine to control infections in accordance with pathogens and drug resistance test results.
