ADA、IFNγ-、sIL-2R检测在鉴别结核性和癌性胸水中的价值 被引量:4

Differential diagnostic value of adenosine deaminase interferon-γand soluble interleukin-2receptors of tuberculous and malignant pleural effusions

摘要 目的研究腺苷脱氨酶(ADA)、γ干扰素(IFN-γ)、可溶性白介素2受体(sIL-2R)对结核性胸水和癌性胸水鉴别诊断的价值。方法采用酶法和ELISA方法检测47例结核性胸水患者和47例癌性胸水患者胸水中ADA活性和IFNγ-、sIL-2R水平。结果结核性胸水组的ADA(72.3±21.7 U/L)、IFN-γ(481.3±257.1 pg/m l)和sIL-2R(611.0±184.4 pmol/L)都显著高于癌性胸水组(P<0.01);当以ADA≥46.8 U/L、IFNγ-≥209.0 pg/m l和sIL-2R≥491.5 pmol/L为临界值,三项指标对结核性胸水的敏感度分别为93.6%,87.2%,70.2%;特异度分别为91.5%,91.5%,78.7%。结论胸水中ADA、IFN-γ和sIL-2R对结核性胸水和癌性胸水的鉴别诊断都有较高的临床价值。 Objectice To study the activity of adenosine deaminase(ADA) and level of interferon-γ(IFN-γ)and soluble interleukin-2 receptors(sIL-2R)in pleural effusions for the differential diagnosis of tuberculous and malignant pleural effusions.Methods Anzymatic method was performed to detect ADA and enzyme-linked immunosorbent assay(ELISA)was to IFN-γand sIL-2R in 47 patients with tuberculous pleural effusion and 47 patients with malignant pleural effusions.Results The levels of ADA(72.3±21.7 U/L)、IFN-γ(481.3±257.1 pg/ml)and sIL-2R(611.0±184.4 pmol/L)in pleural effusion of tuberculous effusion obviously higher than those in malignant effusion(P0.01).When ADA greater than 46.8 U/L or IFN-γ greater than 209.0 pg/ml or sIL-2R greater than 491.5 pg/L were diagnostic of tuberculous effusion,their sensitivity were 93.6%,87.2%,70.2%,and specificity were 91.5%,91.5%,78.7%respectively.Conclusion The determination of ADA,IFN-γand sIL-2R level in pleural effusions are beneficial to identification of tuberculous and malignant effusions.【
