摘要
目的探讨不同基础胰岛素治疗2型糖尿病患者控制血糖的疗效。方法采用便利抽样法选取2008-2010年在南京鼓楼医院内分泌科治疗的56例2型糖尿病患者为研究对象,按随机数字表法将其分为两组,每组28例。一组患者加用中性鱼精蛋白锌胰岛素(neutral protamine Hagedorn insulin,NPH),另一组患者加用甘精胰岛素(glargine insulin,GLA)。采用动态血糖监测系统(continuous glucose monitoring system,CGMS)比较两组之间的血糖控制水平及血糖波动情况。结果 GLA组患者中餐前1 h、晚餐前2 h平均血糖水平与NPH组比较,差异有统计学意义(P<0.01或P<0.05)。GLA组患者三餐后3 h的平均血糖水平均低于NPH组,差异有统计学意义(P<0.05或P<0.01)。GLA组患者在3:00-6:00时间区段内平均血糖水平与NPH组比较差异有统计学意义(P<0.05)。GLA组患者的血糖波动最大幅度(largest amplitude of bloodglucose excursion,LAGE)、空腹血糖上升指数(fasting ascending glucose excursion,FAGE)及标准差(standard deviation,SD)均低于NPH组患者,差异有统计学意义(P<0.05或P<0.01)。三餐前1 h及0:00-6:00时间区段,GLA组患者的日间血糖平均绝对差(mean of daily difference,MODD)水平低于NPH组患者,差异有统计学意义(均P<0.05)。结论 GLA组患者血糖控制较好,且血糖波动幅度小。
Objective To compare the effects of different basal insulin therapies on blood glucose control in type Ⅱ diabetic patients.Methods Using convenience sampling method,56 patients with type Ⅱ diabetes between 2008 and 2010 in the department of endocrinology of Nanjing Drum Tower Hospital were enrolled in the study.All the patients were equally randomized into two groups to take neutral protamine Hagedorn(NPH) insulin and glargine(GLA) insulin seperately.Using continuous glucose monitoring system(CGES),the levels of blood glucose and glucose fluctuations of the two groups were compared.Results Compared with NPH group,significant differences were observed in the average blood glucose levels of GLA group 1 h before breakfast and dinner(P0.05 or P0.01).The average blood glucose levels of GLA group 3 h after three meals were significantly lower than those of NPH group(P0.05 or P0.01).Significant differences were also observed in the average blood glucose levels during the period of 3:00-6:00 between GLA group and NPH group(P0.05).The largest amplitude of glycemic excursions(LAGE),the standard deviation of blood glucose(SDBG),the fasting amplitude of glucose excursions(FAGE) in the GLA group were lower than those in NPH group(P0.05 or P0.01).The values of mean of daily difference(MODD) 1 h before meals and during the period of 0:00-6:00 were significantly lower in the GLA group than in NPH group(P0.05).Conclusion The blood glucose levels are better controlled and the blood glucose fluctuations are small in GLA group.
