摘要
目的 探讨ＣＹＦＲＡ２１１，ＣＥＡ和Ｇａｓｔｒｉｎ 组合动态检测在肺癌诊断，分型分期，疗效观察和预后判断的临床价值。方法 选自本院３１ 例肺癌，２８ 例肺部良性疾病住院患者和２８例本院健康志愿者，应用放射免疫分析法（ＩＲＭＡ）检测血清ＣＹＦＲＡ２１１，ＣＥＡ水平；应用放射免疫～ＰＥＧ法检测血清ＣＥＡ和Ｇａｓｔ．水平。肺癌患者在化疗和／或放疗１～２个疗程前后各检测一次。以本院同位素实验室提供的临界值为依据：血清ＣＹＦＲＡ２１１，ＣＥＡ 和Ｇａｓｔ水平分别为３．３ｎｇ／ｍ ｌ，１５ｎｇ／ｍ ｌ和１００ｐｇ／ｍ ｌ。结果 肺癌组患者血清ＣＹＦＲＡ２１１，ＣＥＡ和ＣＹＦＲＡ２１１显著高于良性肺部疾病组（Ｐ＜０．０１）和健康组（Ｐ＜ ０．０１）；按肺癌组织分型分析，血清ＣＹＦＲＡ２１１，ＣＥＡ和Ｇａｓｔ对各类肺癌诊断阳性率最高的分别为Ｓｑ．Ｃ（８３．３％ ），Ａｄ．Ｃ．（８１．８％ ）和Ｓ．Ｃ．Ｌ．Ｃ．（１００％ ）；就肺癌的ＴＮＭ 分期来看，上述三种肿瘤标记物的水平与临床分期呈一致性改变（Ⅳ期＞ Ⅲ期＞ Ⅱ期）；经化疗和／或放疗后，上述肿瘤标记物水平均有不同幅度的下降，但最能从下降幅度体现疗效的仍然是：ＣＹＦＲＡ２１?
? Objective To explore the clinical significance of CYFRA21 1,CEA and Gastrin (Gast) in diagnosis,classification,typing,commenting therapeutic value and prognosis for lung cancer.Methods The serum levels of CYFRA21 1,CEA and Gast among 31 patients with lung cancer,28 patients with benign pulmonary diseases and 28 healthy volunteers were determined by immunoradiometeric assay (IRMA)and radioimmunoassay PEG,respectively.These critical values of this study were provided by the isotopic department of this hospital.These critical values of CYFRA21 1,CEA and Gast were 3.3ng/ml,15ng/ml and 100pg/ml,respectively.Results Serum levels of CYFRA21 1,CEA and Gast were significantly higher in patients with lung cancer than those in patients with benign pulmonary diseases and healthy volunteers(P<0.01),respectively.In accordance with histological classification of lung cancer,so for diagnosis sensitivity,the serum levels of CYFRA21 1,CEA and Gast were respectively highest(Sq.C:83.3%);Ad.C:81.8%;S.C.C.L:100%).In accorndance with TNM classification of lung cancer,there were similar changes between serum levels of three kinds of markers above mentioned and clinical classification(stage Ⅳ>stage Ⅲ>stage Ⅱ;P<0.05 or P<0.01).After chemotherapy and/or radiotherapy,serum levels of above mentioned markers were decreased to varying degrees.But CYFRA21 1 decreased most in Sq.C;CEA dereased most in Ad.C;Gast in S.C.L.C.And serum peak values of patients with lung cancer and their decreased degrees can predict prognosis.Conclusion The dynamic examinations of serum CYFRA21 1,CEA and Gast have clinical significance in diagnosis,classification,staging,commenting therapeutic value and prognosis for lung cancer. 〔
