曲普瑞林对子宫肌瘤及内膜组织ER、PR、bcl-2、bax的影响 被引量:6

摘要 目的观察曲普瑞林治疗子宫肌瘤的效果及对子宫内膜的影响。方法子宫肌瘤患者(对照组)30例及经曲普瑞林治疗的子宫肌瘤患者(治疗组)40例,术中均取瘤体及内膜,采用免疫组化SP方法检测肌瘤和内膜组织中雌、孕激素受体ER、PR和凋亡基因bcl-2、bax的表达。结果应用曲普瑞林后,肌瘤组织中ER、PR及内膜中ER的表达水平明显下降(P<0.01),而内膜组织中的PR表达变化不明显(P>0.05);bcl-2在肌瘤组织的阳性表达率明显降低(P<0.05),在内膜组织的阳性表达无显著变化(P>0.05);bax在肌瘤组织的阳性表达明显升高(P<0.05),在内膜组织的阳性表达率稍上升(P>0.05)。曲普瑞林治疗后内膜85.0%(34/40)表现为增生期改变,其中76.4%(26/34)为增生不良。结论曲普瑞林能使相应的肌瘤ER、PR减少,同时能促进子宫肌瘤细胞凋亡,又可能造成子宫内膜的增生不良。 Objective To study the effect of diphereline on endometrium after treatment of hysteromyoma with diphereline. Methods The expressions of ER, PR, bcl - 2, and bax in both endometrium and hysteromyoma in control group ( n = 30 ), and treat- ment group ( n = 40) were detected by immunohistochemical S - P method. Results After diphereline treatment of hysteromyoma, the positive rate of ER expressions in hysteromyoma and endometrium in treatment group decreased( P 〈 0.05 ) while the positive rate of PR expressions in hysteromyoma and endometrium in treatment group was not different from that in control group( P 〉 0.05 ). The positive rate of bcl - 2 expression in hysteromyoma was lower than in control group with statistically significant difference, but there was no sig- nificant difference in endometrium between the two groups before and after diphereline treatment. Compared with control group, the pos- itive rate of bax expression in hysteromyoma was increased in diphereline group ( P 〈 0.05 ) , but there was no significant difference in endometrium between the two groups ( P 〉 0.05 ). 85% (34/40) of the patients in diphereline group showed changes of the proliferative phase in endomefrium, 76.4% of whom (26/34) showed simple hyperplasia. Conclusion Diphereline promotes apoptosis of hysteromyoma cells and probably contributes to proliferation of endometrium.
