钙和镁对缺氧性肺、脑血管反应的影响 被引量:4

摘要 目的 以不同浓度氯化钙、氯化镁静脉滴注,观察钙、镁对大肺、脑血管缺氧损害的保护作用。为钙、镁制剂的临床应用提供依据。方法 用低氧复制犬的动物缺氧模型,采用氯化钙、氯化镁每min0.5mg/kg对犬连续滴注5h,在用药前后测定缺氧犬血液动力学指标及血气分析。结果 氯化钙与氯化镁 0.5mg/kg组急性肺泡缺氧时,血液动力学参数肺血管阻力变化率(APVR)与对照组相比显著降低(P<0.01),体血管阻力变化率(△SVR)及脑血流量变化率(△CBF)与对照组相比有显著差异(P<0.05);肺动脉平均压升压反应(△Ppa)和体动脉压(△Psa)以及心输出量(△CO)无明显改变。结论 氯化钙与氯化镁联合应用可提高降低缺氧性肺、脑血管阻力,降低缺氧性肺动脉高压的药物疗效;减少高浓度镁对心率、血压、心肌的不利影响;减少高浓度钙增加血液粘滞度及对心脏的不利影响,且可减少钙制剂和镁制剂的单独用药的剂量,提高疗效。 Objective Calcium and magnesium were observed for the protective function of dog's pulmonary. cerebrovascular anoxic injuries by intravenous drip of different concentrations of calcium chloride, magnesium chloride so as to provide a basis for clinical application of calcium, magnesium preparations. Methods Dod's anoxic models were replicated under hypoxia with continuous drip infusion of 0.5mg/Kg calcium chloride, magnesium chloride every minute for five hours running. Dog's anoxic hemodynamics index, and blood and vital energy analysis were detected before and after administration of the drugs, Result in the groups with 0.5mg/Kg calcium chloride, magnesium chloride during acute alveolar anoxia, pulmonary vascular resistance rate of hemodynamics parameter (△PVR) was reduced remarkably, as compared with that in the control group(P < 0.01). There was obvious difference in the systemic vascular resistance rate(△SVR) and cerebral blood flow rate(△CBF), as compared with that in the control group (P < 0. 05); and no marked difference in average hemopiesic reaction of pulmonary artery(△Ppa), systemic artery pressure (△Psa) and candiac output(△CO). Conclusions The combined administration of calcium chloride and magnesium chloride can increase the drug's curative effect in reducing hypoxia pulmonary. Cerebrovascular resistance, and lowering anoxic pulmonary artery hypertention, depleting unfavourable effect of high concentration, magnesium on heartbeat rate, blood pressure and heart muscle; reducing the harmful effect of high concentration calcium on the heart by increasing blood viscosity and also lessening the single dose of calcium reagent and magnesium preparation so as to improve the curative effects.
