摘要
目的:药师通过联络信,对不合理用药医嘱进行纠正,发挥药师人为干预作用,确保患者用药安全合理。方法:对我院2009年7-11月外科药房静脉药物配置中心的所有医嘱进行审核,收集药师与医师的1058张联络信,采用Excel 2003软件系统进行统计并分析统计结果。结果:1058张联络信按所纠正的医嘱问题分为8种,主要为溶媒不当(50.10%)、药品用量不当(24.10%)和容量不当(9.64%)。随着联络信制度的开展,2009年11月的不合理医嘱数量比7月下降约67.16%。结论:在基于全方位一体化药房的PIVAS中,药师通过联络信发挥专业作用,干预临床合理用药,加强全方位一体化高质量的药品保障,充分体现了药师在医院药学中的作用。
Objective：By examining the prescriptions,pharmacists correct the unreasonable clinical orders,take a part of human intervention,and insure the safety and rationality of drug use.Methods：The clinical orders in pharmacy intravenous admixture services（PIVAS） of the surgery building in our hospital from July to November in 2009 were examined,the 1058 contact letters were collected and the results were analyzed.Results：In terms of the improper causes of the clinical orders,the 1058 contact letters were classified to 8 types,in which the incorrect solvent（50.10%）,the incorrect drug dose（24.10%）,and the improper volume（9.64%） were the main problems.Amounts of unreasonable clinical orders in November 2009 decreased by 67.16% compared with that in July.Conclusion：Based on the pharmacy intravenous admixture services in the integrative pharmacy,pharmacists can play a role by the contact letter in intervening the improper drug use,and promoting rational drug use.
