摘要
目的研究曲安奈德玻璃体腔注射联合激光光凝治疗视网膜静脉阻塞所致黄斑水肿的疗效。方法经散瞳间接眼底镜检查、光学相干断层扫描(OCT)及眼底血管荧光造影(FFA)检查证实为视网膜静脉阻塞所致黄斑水肿患者25例25只眼(其中人工晶体眼3例3只眼),玻璃体腔内注射40g/L的TA0.1mL,待玻璃体腔透明度恢复适合激光光凝时行格栅样光凝治疗,随访3～12个月,观察视力、眼压、晶状体、炎症反应、眼底情况,OCT检查视网膜厚度改变,FFA检查黄斑区毛细血管渗漏情况,如发现毛细血管渗漏情况时行视网膜激光光凝。结果所有患者视力均有不同程度提高,个别出现眼压升高和假性眼内炎以及复发,尚需进一步观察长期疗效和安全性。结论曲安奈德玻璃体腔注射联合激光光凝是治疗视网膜静脉阻塞所致黄斑水肿的有效方法,并能减少复发。
【Objective】To research curative effect of triamcinolone acetonide by means of vitreous body cavity injection combined with laser photocoagulation to treat retinal vein obstruction caused by macular edema.【Methods】Indirect ophthalmoscopy,Optical Coherence Tomography（OCT） and fundus fluoresceine angiography（FFA） confirmed that retinal vein obstruction caused by 25 patients（25 eyeballs） with macular edema（artificial lens eye is 3 cases among them） after mydriasis test.To injects triamcinolone acetonide（TA,0.1 mL 40/L） into vitreous body cavity,then carried up grid laser photocoagulation operation for those patients when transparency in their vitreous body cavity is recovery until their sick eyes suitable for laser photocoagulation treatment.After operation the follow-up time was 3-12 months,whereafter,to detect visual acuity,intraocular pressure,lens,inflammation reaction,fundus situation.Optical Coherence Tomography（OCT） examined those patients,retinal thickness,meanwhile fundus fluoresceine angiography（FFA） examined Capillary leakage condition of macular area,as if capillary leakage we performed retinal laser photocoagulation operation without delay.【Results】All patients,visual acuity has been developed in different degree,there is condition that intraocular pressure increased and false endophthalmitis and macular edema relapsed.These conditions need observe Curative effect and safety in a long time.【Conclusions】It is an effective method that triamcinolone acetonide by means of vitreous body cavity injection combined with laser photoco-agulation to treat retinal vein obstruction caused by macular edema,moreover it can reduce the recurrence.
