血栓前状态的检测对PICC血栓形成的预警意义 被引量:18

摘要 目的:探讨血栓前状态的检测对外周静脉置入中心静脉导管(Peripherally inserted central catheter,PICC)血栓形成的预警意义。方法:通过随机抽取我科30例PICC置管患者,以2009年6月19日~2010年2月22日的临床检验资料进行统计分析。通过对PICC置管前0~1 d(T0)、置管后2~3 d(T1)、置管后4~11 d(T2)与血栓形成相关量化指标凝血酶原时间(Prothrombin time,PT)、凝血活酶时间(Activated partial thromboplsatin time,APTT)、凝血酶时间(Thrombin time,TT)、纤维蛋白原量(Fibrinogen,Fbg)、纤维蛋白降解产物(Fibrinogen degradation products,FDP)及D-二聚体的动态检测,对比T0与T1、T0与T2、T1与T2,观察各时间段各项指标的变化。结果:T0与T1比较,2组PLT值有显著差异(P〈0.05),而PT、APTT、TT、Fbg、FDP及D-二聚体则统计学差异不明显;T0与T2比较,PT、APTT、TT、PLT、Fbg、FDP及D-二聚体统计学差异亦不明显;T1与T2比较,PT、APTT、TT、PLT、Fbg、FDP及D-二聚体统计学差异同样不明显。结论:只有PLT在T0与T1时间段具有显著差异,提示置管后2~3 d检测外周血血小板(Plateled,PLT)在PICC血栓形成前具有预警意义。 Objective:Tostudythe earlywarningmeaningofprethrombotic state test before thrombosis caused byPICC. Methods:Clinical data of 30 patients selected randomly in our department during 2009.6.19~2010.2.22 was analyzed retrospectively. All the parameters brought into our study included PT ,APTT,TT,PLT,Fbg,FDP and D-dimer. Every parameter was dynamically detected 3 times,0~1 days before (T0),2~3 days after (T1),4~11 days after (T2) PICC inserted. Three values of each parameter were compared in pairwise comparison. Results:The difference of PLT between T0 and T1 was significant (P0.05). There was no significant difference in PT, APTT,TT,Fbg,FDP and D-dimer between T1 and T0 (P0.05). Between T0 and T2,T1 and T2,there was no significant difference in all the 7 parameters(P0.05). Conclusion:As the significant difference in PLT between T0 and T1,PLT test before and 2~3 days after PICC inserted may have great meaning of early warning thrombosis.
