摘要
目的探讨食管鳞状细胞癌中P16基因蛋白、Ki.-67基因蛋白的表达水平及与淋巴结转移、预后的关系。方法选择56例食管鳞状细胞癌及其癌旁正常黏膜,应用免疫组织化学染色S-P方法进行P16基因蛋白、Ki-67基因蛋白的检测。结果癌旁正常黏膜中,P16阳性表达率为89.29%(50/56),食管鳞状细胞癌中其阳性表达率为42.86%(24/56),经比较二者差异有统计学意义(P〈0.05);癌旁正常黏膜中,Ki-67阳性表达率为8.93%(5/56),食管鳞状细胞癌中其阳性表达率为33.93%(19/56),经比较二者差异有统计学意义(P〈0.05)。食管癌高分化组P16阳性表达率明显高于中、低分化组(均P〈0.05);食管癌组织分化差者Ki-67阳性表达率高于分化好者(p〉0.05);P16、Ki-67阳性表达率在有、元淋巴结转移组比较,差异均有统计学意义(均(P〈0.05)。术后生存期〉3年者P16阳性表达率57.69%(15/26)显著高于术后生存期≤3年者30.00%(9/30)(x2=4.36,P〈0.05),术后生存期≤3年者Ki-67阳性表达率46.67%(14/30)高于术后生存期〉3年者19.23%(5/26)(x2=468,P〈0.05)。结论P16、KJ-67可作为反映食管鳞状细胞癌生物学行为的客观参考指标,可作为评定食管鳞状细胞癌的进展、预测其转移的重要参考指标。
Objective To study the expression of P16,Ki-67 gene protein in esophageal squamous cell carcinoma,and its relation with lymph node metastasis,prognostis. Methods The SP immunohistochemical method was used to detect the expression of P16 ,Ki-67 gene protein in 56 cases with esophageal squamous cell carcinoma,and its normal paraneoplastic rectal mucous tissues. Results The positive expression rate of P15 in normal paraneoplastic rectal mucous tissues was 89. 29% (50/56), and 42. 86% (24/56) in esophageal squamous cell carcinoma, there was significant difference between the two groups( all P 〈 0. 05) ;the positive expression rate of Ki-67 in normal paraneo- plastic rectal mucous tissues was 8.93% (5/56) ,and 33.93% (19/56) in esophageal squamous cell carcinoma,there was also significant difference between the two groups ( all P 〈 0. 05 ). In well differentiated groups of esophageal carci- noma the positive expression rate of P15 was significantly higher than that in moderately differentiated group,poorly differentiated group( P 〈 0. 05 ) ;in bad progressed group of esophageal carcinoma the positive expression rate of Ki-67 was higher than that in good progressed group( P 〉 0. 05 ) ;The positive expression rate of P16, Ki-67 in lymph node metastasis was significantly higher than that in without lymph node metastasis (P 〉 0. 05). In the exist period after operation over three years groups ,the positive expression rate of P16 57. 69% (15/26) was significantly higher than that in the groups of the exist period after operation non-over three years 30. 00% ( 9/30 ) ( X2 = 4. 35, P 〈 0. 05 ), in the exist period after operation non-over three years groups the positive expression rate of Ki-67 46.67% (14/30) was higher than that in the exist period after operation over three years group 19. 23% ( 5/25 ) ( X2 = 4. 68, P 〈 0. 05 ). Conclusion P16,Ki-67 were objective markers to estimate the behaviors of esophageal squamous cell carcinoma and to evaluate the development and predict the mettastasis of tumors of patieus.
