摘要
目的探讨合并糖尿病对于高血压患者血浆同型半胱氨酸(Hcy)、颈动脉内膜中层厚度(IMT)的影响。方法根据合并糖尿病情况,将高血压患者115例分为两组:高血压组71例和高血压合并2型糖尿病组(合并组)44例,选择年龄、性别相匹配的健康体检者30例作为对照组,测定3组血脂、血尿酸、空腹血糖、Hcy和颈动脉IMT。结果与对照组相比,高血压组和合并组的血浆Hcy水平[(15.5±5.1)、(19.6±10.1)比(8.0±1.7)μmol/L,P<0.05]、IMT水平[(0.14±0.05)、(0.18±0.05)比(0.10±0.04)cm,P<0.01]升高,差异均有统计学意义;合并组的血浆Hcy水平和IMT水平高于高血压组(P<0.01);Logistic回归分析结果显示,除低密度脂蛋白胆固醇、空腹血糖外,血浆Hcy也是颈动脉硬化的危险因素。结论合并糖尿病促使高血压患者血浆Hcy水平升高,颈动脉硬化加剧。Hcy是高血压患者颈动脉硬化发生发展的危险因素。
Objective To investigate the the influence of coexistion of diabetes mellitus (DM) on plasma homocysteine and carotid artery atherosclerosis in essential hypertension (EH) patients.Methods One hundred fifteen EH patients were divided into two groups:EH group (n=71);EH with DM group (n=44).Thirty cases of healthy individuals were chosen as control group.Blood lipids,blood uric acid,fasting blood glucose,plasma homocysteine (Hcy) levels and the intima-media thickness (IMT) of carotid artery were measured.Results Compared to control group,in EH group and EH+DM group,the plasma Hcy [(15.5±5.1),(19.6±10.1) vs (8.0±1.7) μmol/L,P〈0.05] and IMT level [(0.14±0.05),(0.18±0.05) vs (0.10±0.04)cm,P〈0.01] were significantly increased.Plasma Hcy and IMT level in EH+DM group are significantly higher than that of EH group.After adjusted for cholesterol,triglyceride,high density lipoprotein-cholesterol and uric acid,logistic regression analysis showed that,fasting blood sugar,low density lipoprotein-cholesterol and plasma Hcy were associated independently with carotid atherosclerosis.Conclusion Coexistion with diabetes mellitus increases plasma Hcy and exacerbates the carotid arteriosclerosis in EH patients.Plasma Hcy is a risk factor for the development of carotid arteriosclerosis in EH patients.
