摘要
目的探讨应用医用臭氧治疗腰椎间盘突出症的临床疗效。方法腰椎间突出症39例,按照CT/MRI表现椎间盘突出症分为3种,分别采取侧后入路方法及小关节内侧缘穿刺方法,均经皮腰椎间盘内注射浓度45μg/ml的医用臭氧10~15ml,退针至椎间孔附件时,向椎旁组织内神经根旁注入臭氧10ml,按照纤维环是否破裂分组研究纤维环破裂与疗效的关系,另对2例椎间盘突出症手术后腰背综合征(FBSS)进行臭氧治疗的疗效观察。结果 39例臭氧治疗腰椎间盘突出症3~6个月观察显示:根据MacNab临床疗效评价标准,总有效率为82.95%,其中显效20例,42.42%,有效12例,40.53%,无效7例,17.05%,其中FBSS治疗效果良好。对于未破裂总有效率66.5%,破裂组总有效率87.1%,差异有统计学意义。另对2例椎间盘突出伴度滑脱病人进行臭氧实验性治疗研究,6个月后临床症状显著改善。结论 39例共48盘椎间盘病人臭氧治疗效果确切,2例FBSS疗效良好,2例滑脱病人效果显著,椎间盘破裂组臭氧治疗明显优于未破裂组。同时对椎间盘突出症臭氧治疗的适应症及禁忌症做进一步分析。
Objective To explore the clinical effects of the application of medical ozone to the treatment of Lumbar Intervertebral Disc Protrusion.Methods Select 39 cases of Lumbar Intervertebral Disc Protrusion which are divided into 3 categories based on the diagnosis of CT and. Adopting posterior lateral approach and acupuncture on inner margin of small joint successively; inject 10 to 15 ml medical ozone at 45μg/ml through Lumbar Intervertebral Disk;When receding needle to the appendages of intervertebral foramen,inject 10 ml ozone into paravertebral nerve roots.Study on The correlation between anular disruption and curative effects depends on whether the fibrous ring ruptures or not. Observe the curative effects of ozone treatment for FBSS of Lumbar Intervertebral Disc Protrusion of 2 patients.Results With observation of treating the 39 patients with Lumbar Intervertebral Disc Protrusion for three to six months,the total effective rate was 82.95%,among which 20cases showed remarkable effects accounting for 42.42%; 12 cases showed effects to some extent accounting for 40.53%. 7 cases didn't show any curative effect accounting for 17.05%. The treatment of FBSS was effective for the total effective rate is 66.5% for the unruptured group and 87.1% for the ruptured group with remarkable statistical differences. In addition to this,study on the experimental treatment for 2 patients of Lumbar Intervertebral Disc Protrusion with spondylolisthesis,the clinical syndromes were significantly improved after six months.Conclusion Treatment for the 39 patients with 48 Lumbar Intervertebral Disc Protrusions using medical ozone was significantly effective,so did the treatment for 2 cases of FBSS and 2 cases of spondylolisthesis. The curative effect of the Lumbar Intervertebral Disc ruptured group was better than that of the unruptured group. Meanwhile,there should be further analysis on the indications and contraindications of treating Lumbar Intervertebral Disc Protrusion with ozone.
