椎间盘髓核造影在臭氧治疗椎间盘突出症中的应用价值 被引量:4

摘要 目的探讨椎间盘髓核造影在臭氧治疗不同类型椎间盘突出症方面的临床意义。方法经CT或MRI诊断明确的264例椎间盘突出症患者,采用椎间盘髓核造影明确纤维环的破裂情况,将椎间盘突出症分成纤维环未破裂组和纤维环破裂组,其中未破裂组14例,破裂组250例,采用45μg/ml浓度的臭氧气体进行椎间盘内及盘外注射,观察有效率。结果264例343个椎间盘,其中纤维环未破裂组14例(16个盘),纤维环破裂组250例(327个盘),总有效率为82.95%,其中显效112例,有效107例,无效45例。未破裂组有效率42.85%,破裂组有效率85.2%。两组临床数据经方差分析,差异有统计学意义(χ2=17.226,P<0.05)。结论椎间盘髓核造影在臭氧治疗椎间盘突出症方面有着重要的临床参考价值。 Objective To evaluate the clinical value of discography in the interventional treatment of lumbar disc herniation by medical ozone.Methods Two hundred and sixty four patients with lumbar disc herniation diagnosis by CT or MRI were involved in this study.The discography of intervertebral disc nucleus was applied to demonstrate the rupture of annulus fibrosus.The subjects with intervertebral disc herniation were divided into annulus fibrosus unruptured and ruptured group,of which 14 cases in unruptured group,250 cases in ruptured group.Fortayfive ug/ml of ozone was injected within and out of intervertebral discs,and the effectiveness was evaluated.Results There were 264 cases with herniation including 343 discs,of which 14 cases in unruptured group(16 discs),250 cases in ruptured group(327 discs).The total efficiency was 82.95%,of which 112 cases were markedly effective,efficiency in 107 cases,no effect in 45 cases In unruptured group,the effective rate was 42.85%,but 85.2% in the ruptured group.there was a significant statistical difference between the two groups(χ^2 =17.226,P0.05).Conclusion There is an important clinical implication of discography through intervertebral disc nucleus in the ozone treatment of different types of intervertebral disc herniation.
