摘要
目的:探讨呼吸衰竭血气改变与预后的关系。方法:同步测电解质和动脉血气,将血气结果中PH、PaCO2、PaO2各分两组,对其死亡率进行比较,所得数据采用X2检验进行统计学处理,并分析酸碱失衡类型发生率及与死亡的关系。结果:PH<7.25组死亡率明显高于PH>7.25组,两组相比有显著统计学意义(P<0.01),两者呈显著负相关,PH值愈低,死亡率愈高;PaCO2≥9.33kPa组死亡率明显高于PaCO2<9.33kPa组,两组相比有显著统计学意义(P<0.01),两者呈显著正相关,PaCO2高者死亡率高;PaO2≤7.33kPa组与PaO2>7.33kPa组死亡率比较无明显相关性,未显示统计学差异(P>0.05)。90例中呼吸性酸中毒、呼吸性酸中毒合并代谢性碱中毒、呼吸性酸中毒合并代谢性酸中毒发生率及死亡率高,三重型酸碱失衡死亡率高。结论:血液PH值下降和PaCO2升高对肺心病呼吸衰竭预后有十分重要作用,应重视肺心病呼吸衰竭血气酸碱失衡的防治。
Objectivcs:Discuss the rclationship between prognosis and the change of blood gas analysis in patients with respiratory failure.Methods:Simultaneously measuring eleclrolyle and blood gas analysis,according to PH,PaCO 2,PaO 2 in results of blood gas analysis two groups are divided,comparing mortality rate of every group.Qualitative data in every group is examined by x 2 test and analyze the relationship between mortality and type of acid-base disturbance. Results:Mortality rate in PH≤7.25 group is more than than in PH>7.25 group.It is remarkable statistic significance between both groups.( P <0.01).They are negative correlation.Data of PH is lower while mortality rate is higher.Mortality rate in PaCO 2 ≥9.33 kPa group is more than that in PaCO 2<9.33 kPa group.It is remarkable statistic significance between both groups ( P <0.01).They are positive correlation.Data of PaCO 2 is higher while mortality rate is higher.Mortality rate between PaO 2≤7.33kPa group and PaO 2>7.33 kPa group is not correlation.It is not statistic significance ( P >0.05)。Incidence rate and mortality rate of respiratory acidosis,respiratory acidosis complicated metabolic alkalosis,respiratory and metabolic acidosis are high in 90 patients.Mortality rate of triacid-base disturbance is high.Conclusions:Decrease of blood PH and increase of PaCO 2 are very important to prognosis in patients with Cor pulmonale and respiratory failure.We should pay attention to the prevention and cure acid-base distrubance in patients with Cor pulmonale and respiratory failure.
