摘要
的：探讨腰椎间盘突出症手术并发症的防治。方法：分析近８年我科腰椎间盘突出症手术并发症的原因。讨论其防治和治疗措施。结果：术中保留棘上韧带能避免术后腰酸乏力，采用环锯去除椎板黄韧带省时、安全。Ｌ５和Ｓ１的术中确定可依据其形态、活动度、叩击音区别。切除椎间盘的同时，宜探查根管，若狭窄行减压。术中尽可能少牵拉神经根并及时放松。椎管内止血要彻底。结论：严格掌握手术适应证，术前充分准备，术中精细操作，术后认真护理能减少并发症的发生。
Aim:Discuss how to prevent and manage the surgical complications of herniation of lumbar disc.Methods:Analyse the causes of the surgical complications occurred in the last 8 years and discuss the prevention and treatment methods.Results:Preservation of supraspinal ligament in the operation can avoid strain of lumbar muscles afterwards.Using ring saw is safe and fast for removing lamina and ligamentum flavum.The lumbar vertebra can be distinguished from the sacrum by the shape, movement and knocking sound in the operation.Probing into the nerve root canal should be routinized as a part of the whole operation and decompression applied to the canal stenosis.The nerve should be pulled as less as possible.Hemostasis thoroughly can aviod haematoma formation and adhesion.Conclusions:Strictly selected indication,full preparation before surgery,good performance during surgery and careful nursing after surgery will reduce the complications.
