多囊卵巢综合征患者糖脂代谢异常及胰岛素抵抗的特点 被引量:7

Characteristics of abnormal glucose and lipid metabolism and insulin resistance in women with polycystic ovary syndrome

摘要 目的了解多囊卵巢综合征(PCOS)患者糖脂代谢及胰岛素抵抗(IR)的特点。方法纳入137例PCOS患者(PCOS组)及116名年龄匹配的健康女性(Con组),测量身高、体重、腰围;行75g无水葡萄糖耐量试验,并测空腹血清胰岛素、血脂。结果 PCOS患者中41.6%为超重/肥胖,45.4%为中心性肥胖;糖耐量异常、空腹血糖受损、2型糖尿病的患病率分别为26.3%,2.3%,4.5%;血脂异常的患病率为58.6%,主要表现为高TG(23.3%)和低HDL-C(46.6%);IR的患病率为49.6%;以上各指标PCOS组的患病率均明显高于对照组(P<0.05)。亚组分析显示,超重/肥胖的PCOS患者糖脂代谢异常及IR的患病率均明显高于正常体重的PCOS患者(P<0.05)。结论 PCOS组患者具有明显的向心性肥胖特征,糖脂代谢异常及IR的患病率明显高于年龄匹配的Con组,且体重的增加会加重糖脂代谢异常及IR。 Objective To investigate the characteristics of abnormal glucose and lipid metabolism and insulin resistance (IR) in women with polycystic ovary syndrome (PCOS). Methods 137 women with PCOS and 116 age-matched normal women were enrolled in this study. Physical examination included height, weight and waist eircumference. All subjects underwent a standard 75-g oral glucose tolerance test, and fasting serum insulin and lipids were measured. Results The percentage of overweight/ obesity and central obesity in subjects with PCOS were 41.6% and 45.4% respectively. The percentage of impaired glucose tolerance (IGT), impaired fasting glucose (IFG) and type 2 diabetes mellitus (T2DM) in subjects with PCOS were 26.3% , 2. 3% and 4.5% respectively. 58.6% subjects with PCOS had dyslipidemia,whieh was characterized mainly by high TG(23.3%) and low HDL-C(46.6%). 49.6% subjects with PCOS had IR. All percentages were significantly higher in women with PCOS than in control subjects (P〈0. 05). The subgroup analysis showed that the percentage of dysglycemia, dyslipidemia and IR in overweight/obese subjects with PCOS were significantly higher than in normal weight subjects with PCOS(P〈0.05). Conclusions Compared with age-matched control subjects, the PCOS subjects are accompanied by a higher prevalence of central obesity, dysglycemia, dyslipidemia and IR, which increases significantly along with BMI increasing.
