摘要
目的探讨cT瘘管造影在复杂性肛瘘中的临床应用价值。方法对17例复杂性肛瘘患者进行64排CT瘘管造影检查，cT图像进行三维表面重建（3D—SSD）处理。并与术中所见进行比较。结果cT瘘管造影及其后处理影像可以清晰显示复杂肛瘘瘘管的分支系统。在其指导下行肛瘘根治术的患者，经过至少1年随访，治愈率100％。结论CT瘘管造影结合三维重建技术可以指导复杂性肛瘘的临床手术治疗，是提高手术治愈率的重要诊断方法，值得进一步推广。
Objective To investigate the clinical application of spiral CT fistulography in diagnosis of complex anal fistula. Methods Spiral CT fistulography and X - ray visualization were performed in 41 patients with complex anal fistula. The CT image was processed with 3D SSD. The clinical value of the two visualization technique was compared in combination with operative observation and follow - up data. Results Spiral CT fistulography with three - dimensional post - processing techniques could clearly show the origin and branches of complex anal fistula in all cases. According to the CT fistulogra- phy, anal fistulotomy was performed in all the patients. The cure rate reached 100% after following - up at least for one year. Conclusion Spiral CT fistulography with 3D post -processing helps direct the clini- cal operation of complex anal fistula and increase the cure rate.
