摘要
目的对比分析河北省赞皇县胃癌高、低发区居民血清胃蛋白酶原、胃泌素和幽门螺杆菌抗体变化，以探讨这些指标与胃癌发生是否有关。方法以放免和ＥＬＩＳＡ方法对赞皇县胃癌高发区和相对低发区９２９名居民的血清胃蛋白酶原Ⅰ、Ⅱ、胃泌素和血清幽门螺杆菌抗体进行了对比分析。结果赞皇县胃癌高发区和相对低发区居民血清平均胃蛋白酶原Ⅰ、Ⅱ、Ⅰ／Ⅱ比值的中位数分别为５４．１μｇ／Ｌ、１２．９μｇ／Ｌ、４．４和５４．５μｇ／Ｌ、１２．３μｇ／Ｌ、４．５，两地区无明显差异；胃癌高发区居民血清胃泌素水平明显高于相对低发区（８５ｐｇ／ｍｌ∶６８ｐｇ／ｍｌ，Ｐ＜０．０１）；两地居民幽门螺杆菌感染均较普遍，胃癌高发区和相对低发区居民血清抗幽门螺杆菌抗体阳性率比较，差异无显著性（Ｐ＞０．０５）。结论除胃泌素外，赞皇县胃癌高发区和相对低发区居民血清胃蛋白酶原Ⅰ、Ⅱ和血清幽门螺杆菌抗体水平均差异无显著性（Ｐ＞０．０５）。
Objective To study the possible differences in the serum pepsinogen, ,, gastrin and Helicobacter pylori antibody levels between the residents of high and relatively low risk areas of gastric cancer in Zanhuang County. Methods Serum pepsinogen & , gastrin and Helicobacter pylori antibody levels were studied in 929 residents from the high risk area (H area) and the relatively low risk area(L area) with radioimmunological assay and ELISA method. Results The results showed that the median serum levels of pepsinogen ,,/ ratio for H and L area were 54.1g/L, 12.9g/L,4.4 and 54.5g/L,12.3g/L,4.5 respectively. There were no differences in the above paraneters between the residents of H and L area. The median gastrin level of those in H area was significantly higher than that in L area(85pg/ml vs 68pg/ml,P<0.01). Helicobacter pylori infection was quite common among the residents of the two areas and no significant difference in its seropositivity rate was found between H and L area. Conclusion Except for gastrin, there were no significant differences in the serum PG, PG,PG/PG ratio and anti Hp antibody levels between the high and relatively low risk areas of gastric cancer in Zanhuang County.
